Date: 6/4/24 Amount: $5614 Committed to completely resolving my back pain through treatments. Dispute: They took complete advantage of me. They sped through the "sign up" papers without explaining what they were, had the screens zoomed in so you couldnt see what you were signing, and it turned out they were fraudulently applying for a care credit card in my name and charging it for over $5k. This is extremely poor business practice- borderline fraudulent. They also over exaggerated what I had going on with his back - this over exaggeration confirmed by a 2nd opinion of another chiropractor. After being charged all that money, going through the treatments, at home exercises, etc they guaranteed that I would be better and I am absolutely not better.They offered to give treatments for myself and any of my family until back pain was resolved. But this is unsatisfactory and I will not be accepting that offer.Business response

Im so very sorry to hear about your husbands experience, although there are some things that are not very clear or accurate about the experience. Our office is extremely clear about expectations in care, pricing and expected results. So much so, that we include in multiple documents before even coming into the office that there are no guarantees in any practice of medicine at any facility, as well as going over our insurance and billing policies (which upon checking, he signed those documents before stepping foot into our facility). After reviewing his case, he was given options for treatment and the associated cost for each, all on a paper document that he also signed. He also signed a paper document to check and utilize care credit for the option he chose. He also signed a paper receipt from care credit stating the terms of his payments. He then signed an additional care plan that stated the total price and that he was utilizing care credit. After coming in for 4 months of care, he never mentioned any sort of miscommunication or misunderstanding with any part of our processes. Upon checking, he also stated during his re-examination that he was feeling about 50% improvement. We will make sure to fully review the process again to see where we took advantage of him and could have been more clear in the multiple documents he signed.Initial Complaint

Date of transaction: 09/16/2024 Amount of money we paid the business: $8973.00 + $49.00 What the business committed to provide you: They committed to treatment for neuropathy in my right hand.Nature of the dispute: We were misled into signing documents on an ipad and we were not given copies upon signing that day. The documents signed were blown up on an ipad screen and we were only able to see the "sign here" line. We were told by Dr. ******** that treatment we were signing up for would cost $200/month with the option of opting out at any time. We were never told that we were signing up for a credit card. Upon receiving a Care Credit bill for the amount of $8973.00 (which we were NEVER quoted) we promptly returned all equipment and unopened supplements and opted out immediately. Whether or not the business has tried to resolve the problem: After receiving the Care Credit Card bill in the mail, we notified them immediately that we did not open a credit card with them. Then proceeded to call Revival Health and Wellness to rectify the situation with no resolution. At that time, we were given the signed documents and realized that Dr. ******** had misrepresented exactly what we were signing.Business response

I went to Revival because they had a sign saying they can help with Gut health. I have allergies to garlic and onion and they told me they could help me. I told them all the foods I have issues with. They gave me a bunch of supplements and a list of foods to eat. The foods had garlic and onion in them so I couldn't eat most of it the supplements had garlic and onion families in them, I could not take them. I said many things to them about this, then, I got a letter from CareCredit saying that the amount I applied for was denied and I got a bill for $6,281.00 which was maxed out on this credit card that I had NO IDEA that Revival applied to fraudulently in my name. I asked for a breakdown of charges to which I never got. I asked for a refund and they refunded a portion of the amount. They need to refund the full amount as I never agreed to opening a credit card. Now the company has closed it doors in ****** where I went. This company is a Fraud.Business response

We apologize that the signed receipt acknowledging the total amount of cost, payment term, and care credit written at the top was not more clear that the patient was utilizing care credit for the signed terms they agreed upon. I have reviewed the business' response and am rejecting it because:Thank you for sending me this. However, this is not something I signed. I signed a computer screen that was supposed to be for treatment, that is what they said. You do not see this on the screen. They can take my electronic signature and put it on anything they want. This is beyond belief.Tell them I want a break down of these charges so I can take them to court.

You initially enrolled in a 14-week treatment plan, which included in-office treatments and a customized at-home nutrition protocol with a supplement regimen. As part of this program, you received your custom nutrition supplements, two specific therapeutic IV therapy sessions, and Red-*************. When you decided to discontinue the program early, we adjusted your account to reflect the services and products provided, refunding the remaining balance. At no point were you charged for the full treatment span beyond what was delivered.
We apologize if the refund process or the terms of the agreement were not clear. However, by signing the agreement, you acknowledged that you understood the terms and conditions, which are in place to ensure full transparency and fairness for all our clients.

I got an x-ray for $49.00 then I went in for consultation the next day and they write down super fast. I was in so much pain and was an easy target. They sold my number to everyone else. During your consultation and report of findings, our team thoroughly reviewed your condition and presented treatment plan options. You signed all necessary documents acknowledging that you understood the recommended treatments, the associated costs, and the financing options. These documents were provided to ensure that you were fully informed before moving forward with your treatment.
The pricing for the care you received was clearly outlined and transparent from the beginning, and you signed documents acknowledging your understanding of the treatment costs and financing arrangements.
Regarding your concern about your contact information being shared, we want to clarify that we do not sell, share, or distribute any client information. Any communication you may have received would have been directly related to your treatment plan or financing agreement.
After completing your initial 32 visits, you chose to sign up for additional care and continued receiving treatment at our office. This is why you have two charges. If you were dissatisfied with the care provided as well as any discrepancies in our office procedures, we are uncertain why you would choose to proceed with further treatments in our office. Started a program to help my right knee pain . Paid $1000.00 cash up front and financed $5100.00. Never was a time and amount of appt. To get to where I needed to be better was revealed . I went July 2023 through September 2023. No pain relief was done at least very little at first then it got more soar as time went on and I couldnt even finish a 3 day work week 5 hours a day. So I went to an orthopedic surgeon and he said I will never get relief that way ( through a program like I was on) because I am bone on bone which Revival knew up front. So he gave me an injection in my knee to get me by for a few months. I call revival to let them Know this wasnt working after Two months and I need to stop because I am losing work and it Is more sore than ever , ***** said he Could do and injection I said no I just had one and I just wanted to be done and if he could just prorate a refund for the unused portion of the program. He said he couldnt because I was close to being done. I was shocked and said I had only been going for two months and that this was a scam / robbery. I just want a refund (prorated) for the unused portion of this program.Customer response

Managing knee pain can be complex, and our treatment programs are designed to address underlying issues rather than simply masking symptoms. We would like to respectfully disagree with the other provider, stating that our services would not be of benefit for knee pain conditions. At the start of your program, we conducted a thorough evaluation, during which we discussed your condition, healing times, expected side effects and the treatment plan. Our goal was to provide therapies tailored to promote healing and improve knee function over time. By setting the timeline, we would not expect full results in the abbreviated timeline that you were expecting.
During your time in the program from July through September 2023, you received the services outlined in your treatment plan. When you reached out to express concerns about your progress, we discussed the option of a different type of injectionnot the same as the one you had already received from your orthopedic surgeon. This alternative was designed to support the healing of the injured area, not just to mask pain.
When we attempted to follow up with you via text message, you mentioned that you were working on a roof and would get back to us, but we did not hear from you afterward. I am almost 70yrs old. April 10,2023 I became a patient at Revival Health and Wellness. Fenton Mo I went with excruciating pain from spinal stenosis after seeing their ads on social media. I had tried other means to help and it didn’t work. After an interview with *** **** ********* and ** ***** ******** along with my x-rays. They gave me hope that the treatments of spinal Decompression and Soft wave would help. When I asked the cost, Dr. McDaniel went out of the office and came back with a paper, listing three tiers of costs. Tier 1 was Both decompression and soft wave treatments $12,817 He said my ins. would pay $5,243 My cost was $7574 for 24 treatments with Dr evaluations. ( $7,195 if paid in full. ) I told him I would have to finance. A woman at the desk immediately sent me the email to the finance company that they went through. I was accepted for the loan for $7574. (I didn’t ever get the paid in full amount of $7,195) After the first two decompression treatments, I told the Dr McDaniel that I was in more pain than when I arrived there. He told me it was just like exercising for the first time and then I would be sore. I told him I was in pain, not sore. After each treatment, he continued to tell me that it would take a while for me to have any results. I continued telling him that the pain was worse. After telling me, I would get better I believed him each time. After 11 decompression treatments, and.4 softwave treatments. Finally, I realized that I was a fool to believe him. I told him I wasn’t coming back anymore and he still gave me excuses for my pain. I got a refund of $2315 for the remaining visits. I finally got an itemized statement after asking 4 times for it. I was $2489 for a Rebuilder and Dr fees. $450 for nutritional plan, which consisted of two containers of powder. When I asked for my money back he smiled and said that I wasn’t getting it back! I want my money back. I was in more pain than when I started.
