    The date of the transaction was 4/23/21. My chiropractor ordered a diffusion *** of my lumbar spine on 4/20 of 2021. I have been battling severe health issues for several years now and this is the reason why he wanted the diffusion *** of my lumbar spine. This is not a typical *** so most places don't even offer it in the ******** area. I called over to professional imaging and asked them if they could do it they told me to come over and they thought they would be able to do this type of *** it's usually done on the brain and not the spine. I went over to their office on 423 2021 show them a picture of all the health issues that I have been dealing with over the past several years. Originally they told me they wouldn't be able to do the diffusion *** and then after seeing the picture of everything that has been going on they called the radiologist directly and asked him what he would recommend as far as an *** and he told them he could do the diffusion *** and that's what they told me so I agreed to do it. They performed a regular *** along with a diffusion *** all at the same time. The technician came over the speaker and told me that they were going to start the diffusion part of the *** and as soon as it started my legs and arms started shaking uncontrollably bouncing off the table the *** tech was notified through the speaker system she witnessed it. I then called my chiropractor and the facility around 3:00 p.m. to notify them of what happened and to please put all this information in my chart. After I see the CD and the radiologist report I noticed that there was no information concerning the diffusion part of the ***. I went back the next day to their office and asked him why this information isn't on the *** she told me all the information that they can give me is on the radiologist report that's all they can do. They gave me a second CD because the first one was so blurry on the diffusion part of it I couldn't even read it on my computer. There is much more

    Good morning, I have several pictures that I would like to send to you via email which are very pertinent to my case against professional imaging. I was not able to send with the complaint letter. These are all diffusion *** images. I have sent all this information to professional imaging along with several emails in contact with ****. She was supposed to respond back to me for a phone call we had in February 2024 discussing this issue she never got back to me. I have lost my ability to walk now five times. I have other information that I would like to send to you per email if you can provide me with a direct email address. This company did not provide me the service in which they told me I was getting thus committing fraud and medical negligence. I know suffer with severe convulsive tremors throughout my entire body and I have been told by several doctors I can no longer do any type of testing because of these tremors. They are like a grand mal seizure but I don't lose consciousness. I've been told by many doctors that they have never seen anything like this I was also told this a professional imaging by their *** tech. I also have letters that I sent to them back in 4/21/23 per email telling them what happened during the *** and to make sure they put it in my medical records which I'm sure they haven't done. I have been told by doctors if I need any type of testing or if I have an emergency they won't be able to do anything they'll have to put me on life support to do anything because my body has these severe convulsive tremors. You will need all of this information and if you see the *** images that I can send you you will be shocked at what you see. Thank you so much for your help!
