    I had a membership with the Massage Envy located at **** ********** *** *** ****** ** *****. I continued to have billing and scheduling issues with this company and went on location to cancel my membership. I signed all needed paperwork and thought it was taken care of, but later while I was at work 2 men came to my home and scared our 14 year old daughter. They were demanding a payment of $110.00 on a past due bill for Massage envy. I called them and paid to balance request to avoid any additional fear being imposed on my daughter. I have all documentation that proves I cancelled this membership in person on October 9th, 2022.

    To Whom it May Concern: 

    Here is our account and documentation regarding **** ****. 

    Her wellness agreement is attached that she signed when she started her membership, signatures stating that she understood our policies and procedures. 

    The second document attached is her transaction history and audit- highlighting the two different appointments that she owed us for. 

    The third attachment is our screen shots of all of our text threads explaining to her what we were trying to collect, and why, and what would happen if we didn't (police report). We gave her multiple warnings with ample time that we would have to file a police report for walking out after her service and balance. 

    She reached out to us after the police collected her payment. She was mad, and we told her that we texted her multiple times telling her we would have to if she did not call for payment. Her transaction history and audit were provided to her as well. 

    Her membership was terminated immediately when she was in non payment for received service and had a balance owed.
