      I HAVE BEEN A PATIENT OF THIS CLINIC FOR SEVSERAL YEARS NOW ACTUALLY WITHOUT ANY ISSUES UNTIL I STARTED SEEING MANY DIFFERENT NURSE PRACTITIONERS FROM DIFFERENT OFFICES FROM *******,**. TO ********* **, THAT WOULD PERFORM SERVICES AT THE ******* LOCATION AT **************** AS A PATIENT TO THIS LOCATION I HAD NO PROBLEMS UNTIL LATE LAST YEAR WHEN I WAS TOLD BY NP ************************* STATED TO ME THESE EXACT WORDS SHE'S (HER NURSE) HAS COUNTED YOUR BOTTLE AND I HAVE TO VOID YOU THIS IS ALL THAT I WAS TOLD CONSIDERING THAT MY BOTTLE WAS NOT EMPTY I AM YET TO RECEIVE AN EXPLANATION AND HAVE YET TO RECEIVE MY MEDICATION IN WHICH I'M A CHRONIC PAIN PATIENT SINCE MY LAST VISIT IN NOVEMBER 2021 WHEN I WAS VOIDED FOR WHATEVER THAT MEANT I HAVE STARTED BACK AT THIS CLINIC FEBRUARY 2022 TIL PRESENT STILL WITH NO MEDICATION, MAY 11,2022 I HAD A CLINIC APPOINTMENT WITH A NP OUT OF *******,** ************************************************** THIS IS AFTER A NERVE BLOCK BY ******************************************** WITH WHOM HAS ALWAYS BEEN A CLASS ACT WITH MY TREATMENTS, AS WELL AS A VISIT WITH NP. *********************** ON MARCH 30,2022 THE 2 VISITS WITH THE NP'S BOTH ONLY LASTED ***** MINUTES IN WHICH THEY ONLY TOOK MY WEIGHT ,BLOOD PRESSURE AND HEART RATE NO EXAM A FEW QUESTIONS AND SETUP FOR MY PROCEDURE SUPPOSEDLY SINCE MARCH 11TH TO MY APPT. ON JUNE 13TH IN WHICH ON THESE DATES A PRESCRIPTION WAS SUPPOSE TO HAVE BEEN SENT TO ********* AT ******* AND ***** RD. FOR SOMETHING CALLED TRAMADOL IN WHICH I'VE NEVER HAD NOR HEARD OF AND AS OF THIS DAY JUNE 27,2022 ********* HAS NOT BEEN CONTACTED BY MID SOUTH PAIN TREATMENT CENTER I HAVE NOT HEARD BACK FROM THE CLINIC MANAGER ************************* WHOM I'VE CALLED AND LEFT MESSAGES AND MY NUMBER AT LEAST 7 TIMES STILL NO RESPONSE I'VE REQUESTED TO SPEAK TO THE ***** ********************************* WITH WHOM AS ***************** HAS ALWAYS BEEN A CLASS ACT WITH MY TREATMENTS AT THE SURGERY CENTER BUT AS FOR THE CLINIC MY THOUGHTS ARE THAT IT TRULY NEEDS SOME LOOKING INTO AS I'M SEEKING TO FIND MEDICAL HELP SOMEWHERE ELSE.
