      I SET-UP AN AUTOMATED RECURRING PAYMENT ON 4-28-21 W/ LABCORP FOR THE REMAINING BALANCE OF $100 ASSOCIATED WITH AN INVOICE I HAD TO GIVE MY INSURANCE COMPANY TIME TO MAKE ANY FURTHER ADJUSTMENTS ON THE INVOICE. WHILE I WAS SETTING UP THE ARRANGEMENTS ON 4-28-21 LABCORP'S COMPUTER SYSTEM WENT DOWN BUT THE REPRESENTATIVE ASSURED ME THAT IT WAS SET-UP ON MY DISCOVER FOR $50 TO COME OUT ON 5-15-21 & $50 TO COME OUT ON 6-15-21. I MOVED AT THE END OF JUNE. AFTER SETTLING IN I WENT ONLINE TO SEE ABOUT PRINTING A ZERO BALANCE LETTER ONLY TO SEE THAT IT STILL SHOWED $100 OUTSTANDING. I IMMEDIATELY PAID THE $100 BALANCE ONLINE TO LABCORP AND PRINTED THE PAYMENT CONFIRMATION. ON AUGUST 15, 2021 I RECEIVED NOTIFICATION FROM MY CREDIT MONITORING SERVICE THAT THERE WAS A COLLECTIONS ACCOUNT BEING REPORTED FOR $100 BY RADIUS. I HAD NO IDEA WHO RADIUS WAS AND MY ACCOUNT SHOULD NOT HAVE EVER BEEN SENT TO A COLLECTIONS AGENCY. THE COLLECTIONS ACCOUNT NEEDS TO BE REMOVED/DELETED.

      I had services rendered on 3/8/21 with LabCorp with a charge of 72.89. I paid for this directly through ***** on 3/23/21. I have now had 4 bills sent to me, one stating it’s pst due and my final notice. I have spoken to 3 different Lapcorp representatives who state my bill has been posted and taken care of and I now received a statement that I am being sent to collections. I was reassured this was taken care of through the last representative and I would not be sent to collections and here I am. I have exhausted all of my options for LabCorp to post my payment that I had already paid now almost 5 months ago.

      8/4/21 An insurance claim was filed for a health physical and lab work completed. I was given a 3-5 business days turn around. We are well part that time frame and the labs aren't completed. I'm unable to participate and at risk for losing my scholarship. When I call I am given the run around.

      This letter is in response to your correspondence dated August 16, 2021. Laboratory Corporation of America
      (“LabCorp”) appreciates the opportunity to review Mr. ********’s concerns as we take all customer inquiries
      seriously.


      With regard to Mr. ********’s concerns regarding his specimen collection on August 4, 2021, we were able to
      confirm with our laboratory customer service department that the specimen collection for one the tests requested
      had to be forwarded from our laboratory in Phoenix to our laboratory location in Burlington, NC for processing.
      We apologize for any inconvenience this matter has caused. Our records indicate that Mr. ********’s specimen
      collection has been processed and that his test results were reported to the ordering physician on August 16, 2021.

      This complain is for Invoice ********, $308.65. I have been a patient of Dr. ***** *******, MD, in *******, Texas for more than 10 years. His office usually orders labs for my Thyroid and other lab panels every three months or so. In January 21, 2021, I had orders for blood work in his office, I presume they accidently sent my labs to LabCorp instead of Quest because I got a bill from LabCorp. I called the Drs. office and they told me they were going to address the problem, which they never did as I keep getting bills from the LCA Collections. I talked to LCA regarding my situation and they understood that it was not my mistake, however, I still get billed. I called my insurance and they said that if it was rebilled they could adjust it but it was never done. I should not have to pay for something that I have no control over since I do not deal with lab order or billing. I ask that this bill gets adjusted and remove me from this collection agency.

      I received a FINAL NOTICE from Labcorp on 6-29-21 for $18.63. This was my FIRST bill from them. I immediately paid it by an online check through my ***** account. I have confirmation from ***** that it was paid. I received an extortion letter from Labcorp for the same amount, same invoice number, threatening reporting me to a credit bureau if I didn't pay immediately. The telephone number provided is a recorded line that only allows for payment. There is no facility for speaking to a human being to resolve this issue. Given the number of complaints on the BBB website regarding fraudulent practices by this company, isn't there a government agency that monitors misconduct of healthcare providers? How to I reach someone to resolve this issue?

      This is an ongoing dispute where the company has not filed with my insurance as confirmed by my insurance and my EOB statements, They have sent forms seeking more information which we have provided twice and then again in a followup letter, yet LabCorp still has not filed. Please see following documentation. It is now in Internal Collections with LabCorp and I am not getting anywhere, I had insurance during the date of service, this is not the first culture sent from this Doctor during the year with no problems. I also have from ***** My Explanation of Benefits for the month 12/1/20 through 12/31/20, which also shows that nothing had been filed if you need more documentation.

      On April 5, 2021 I received routine medical service from Northside Hospital also know as Atlanta Cancer Care. I received an invoice from LabCorp via email in the amount of $199. Keep in mind, I receive routine medical care on a bi-monthly schedule. I have been bill for services before and after April 5 with no issues whatsoever. I have emailed, faxed, called, sent postal mail and then a certified return receipt letter explaining that the charge is to be paid by the same insurance carrier I've had for two years now. On the invoice, the address is over six years old. The insurance carrier? Same thing. It's from SIX YEARS AGO. I cannot convey the message that my insurance is still the same insurance they have been successfully billing over the past 2 years for all of the other invoices submitted. It's been three months of back and forth with no resolution. I just want them to send the bill to the correct insurance company, and it will get paid. LabCorp refuses to take any action.

      I am a participant in a monitoring program that uses LabCorp for random monthly drug testing. On July 12th I went in for a hair test. I received a chain of custody form with a specimen ID number * * * * * * * * * *. I used the LabCorp at 12114 Cortez Boulevard Brooksville Florida 34613. I was told that the hair test was positive. Upon further review, the website I use to look at my results has a different coc #from the specimen ID that I have on my chain of custody form. The response from LabCorp was that the specimen ID number was already in use and had to create an accession number. This error is costing me over $1,000 in additional testing and evaluation by an addiction specialist and possibly an added five years to my monitoring contract. LabCorp will not speak with me directly because I am not their client Affinity Health is the client they have accepted their response and are no longer investigating this issue. This error is turning my life upside down and no one will listen.
