  My mom was admitted for Rehab Services, my mother does have dementia, I am unhappy with this facility and the case manager who was very unprofessional in handling matters with my mom. 1. When my mother was being discharged, the case manager over my mom DID NOT consult with me before making a decision for my mom . 2. It was stated to me that my mom would need 24hr care until I relocated from Kansas City to Las Vegas which was a total of three weeks. 3. My mother has coverage for home health care, the case manager DID not bother to submit a PRIOR AUTHORIZATON to my mother's insurance company, instead she decided on her own to call a home health agency in which my mother had to sign and pay $2590 up front, which my mom paid for in cash. 4. The physician who treated my mother DID NOT submit a PRIOR AUTHORZATION for 24hr home health care also. 5. Prior to this I left messages for my mom's case manager who was difficult to reach until she had to reach me. 6. When I reached out to my mom's insurance company, that is when I discovered my mom had coverage and not the case manager or the attending physician submitted any forms for PRIOR AUTHORIZATION for her home health care. 7. The case manager and her home health agency decided on themselves to have my mother sign a contract and pay $2590 up front without involving me at all. I expressed to the care manager and the owner of the home health agency I was very unhappy with their process and it was also very unprofessional especially since my mother has dementia. 8. This needs to be resolved because now my mom has paid for 2 weeks cash which she definitely cannot afford due to her case manager not trying to reach out to my mom's insurance which could have saved my mom some money. I would never recommend this facility due to the unprofessional handling and the advantage I feel that was taken of my mother and if I find I can pursue this further I will. BE Aware!!
