Hello, My name is ****** ****. I am contacting you because I don't know who else can help me justify myself of wrongful accusations with a payment of over $2,000 that I refuse to pay. On the 12th of this month, I received a bill from NorthTowns Cardiology 190 maple RD Amherst NY 716 ******** ***The bill states that the 24 hour heart monitor was not returned.*** I called the billing office and spoke to the billing management. He was no help at all, and I became very frustrated. I ended my call with him, and I showed up to the building the next morning to see if I could get any help. The receptionist went back, and some Doctor agreed to talk with me. We sat in a small room off of the offices. The Doctor came into the room and showed me the heart monitor. I said wait......the bill states it's for the heart monitor not being returned. and he stated well that could be a problem because it was damaged by you or the previous user. I said wait you just said that the previous user could have damaged it and you're just pinning it on me. I used the 24-hour monitor and returned it the next morning on time undamaged and now I receive a bill stating that it was not returned when it was but now, they are stating that it was damaged by me or the previous user. I asked the doctor how else I am to prove that I am not lying here get a lawyer? and his response to me was well before you go and lawyer up you should keep in mind that we are a huge company, and you are just one person. I am mortified that this is happening. I hope that you can help me or lead me in the right direction. Please help an honest girl out.

Attached is the Holter Monitor Acknowledgement form that this patient initialed and signed on 08/17/23, BEFORE taking the monitor home with her. This signed agreement clearly states that the monitor is on loan and the patient will be held responsible to repair OR replace the monitor if lost OR damaged. All devices are inspected prior to going out and again upon return. All devices are also cleaned and sanitized prior to going out. This monitor was in working condition when placed on the patient, otherwise we would not have been able to calibrate it and send her home with it. This monitor was returned to us in non-working condition with a crack in one side and a brown, sticky, residue on it. See attached photos. The monitor would not even turn on when returned, we couldn't retrieved any data off of the device from the patient wearing it. Due to this device being completely unusable and non-working, the patient is responsible for the cost to replace the device, as per the loan agreement that she acknowledged with her initials and signature.
