    I was referred to Dental Specialty Associates by *** DDS for a night guard (Code# ****** which is covered by my dental insurance, however, Dental Specialty told me that the insurance would not pay for D9944, so I had to pay out of my pocket ($250.00) on May 16, 2023 when I was at Dental Specialty for an appointment. After I told them that the insurance informed me that it was covered, they told me that they still had to get a pre-authorization from my insurance.More than three weeks later, I found that Dental Specialty had never tried to get pre-authorization but they had received money from the insurance. It looked like Dental Specialty attempted to get money for D9944 from both the insurance and me. Also their hygienist cleaned my teeth very poorly and staffs were not good, so I decided to get my x-rays and go to another dentist. When I requested them to send my x-rays taken on May 16, they told me that I needed to go to their office. When I went to pick up my x-rays on June 8, 2023, they put the x-rays in the envelop and handed it to me. I did not know it was NOT my x-rays until I got home, so I called them however, they kept hang up on me. I reasonably assume that they gave me someone else's x-rays to me on purpose. I have attached the x-rays they handed me here. I would like you to have Dental Specialty e-mail my x-rays which taken on May 16, 2023 to me. Thank you very much,*******************************

    I originally came into the office on 6/9. They claim to the insurance was submitted on 6/14. The last communication I had with the business was that they will get back to me once they receive the information back from the insurance regarding approval and any additional out of pocket cost. I called 5 times. And had been passed only to various departments, a line in which no one answers, or told they would call me back. This has went on for going on 5 months. I have physically walked into the establishment inquiring about the lack of communication, poor business practice and lack of professionalism. It almost start to feel as if they pick and choose which patience they want to response to. I requested to speak to the manager on duty. The building manager *************************, walked passed me 5 times avoiding to stop to talk to me to address the complicate. The receptionist informed me that she has texted him, informing him I was waiting for him. He read the text message in eye sight and still refused to stop and speak to me or at the least schedule a meeting or call.

    Im requesting a refund because my daughter no longer use the service. Multiple times I reached out directly to Dental Specialty Associates no one got back to me. I submitted my request on 4-25-2022 in person for reimbursement with instructions someone will contact me. Im only one initiating contact not receiving no results.
