      Dental services were rendered including a filling and exam on 15 August 2024. I was charged the non-insurance rate even though we had discussed that my insurance was in progress of being reinstated with an effective date of 1 August 2022. Despite the practice saying they would 'work with patients' and that I had just spent approximately $4,000 with the practice ******* the past 4 months, they demanded I pay the full amount before leaving and said that I would be reimbursed for the over charge. My insurance issue was resolved in a few days, and is currently active with coverage as of 1 August 2024 (as expected). I have been to the office multiple times since then as well as emailed and texted. I get no response to email and text inquiries. When i go into the office, the people up front tell me that they have submitted and/or contacted 'billing' to resolve the issue. They have no status updates and will not provide me with any contact information or someone to speak with.

      On Friday, June 21, 2024 I went to the dentist office for a dental reason once the dentist checked my mouth without doing any work he told me to sit and wait for the person from finance to come in. Once she came in. She stated that I had to pay $735 in cash, I asked if I could get the *** code, the diagnosis code and the *** number which they use to bill the insurance. so I could call the insurance to verify whats going on and why I have to pay. (I will have that information with me.)I called my health insurance and they stated that am completely covered. There was no reason for me to pay at all , I went back to speak to the same person and I was told that she had left for the weekend which was impossible because I just spoke to her five seconds ago. They knew that I caught on to what they were doing. They are charging health insurance and also charging patient cash.. as you could see with the information that I submitted thats the bill statements and the *** code which are letters , ALL *** code are in numbers never in letters.. when I asked for the *** number of the doctor and to the facility lady from the finance department stated that she didnt know what was that so how you was charging the insurance with the *** number..

      All procedures were itemized for billing and patient's understanding, in fact her insurance company pays in full for porcelain fused to metal crowns, however it is our responsibility to give the patients the choice of treatment regardless of insurance benefits, the additional $735 was for an upgraded crown that insurance does not pay for. Patient would decide which one to chose. If patient choses porcelain fused to metal instead of all porcelain CEREC crown, that is what we would do, once review and consented by patient. What she was presented was a proposed treatment plan, this is not a statement of services rendered. I attached a copy of the complete treatment plan which includes all the details.

      I have for a few years now have attempted to obtain my dental records. I am now having a problem with an implant done through Concerned Dental. My current dentist requires information, I have been unable to obatin.

      I received a bill for ****** that I believe is an error. I use Aflac dental insurance for many years and have always paid any costs that were not covered out of pocket at the time of service. I upgraded my plan through my work and I believe that is where the error occurred. I have not been able to get ahold of Concerned Dental. The billing ***** never answers the phone or returns my messages. I have my aflac agents calling and trying to resolve this issue as well and they have not been able to get a hold of them either. They. have faxed the information showing my coverage and they go un answered. The bills threaten being sent to collections if not paid in full in 10 days. I have never received a bill or warning from them before. It has been one month of trying to get ahold of them. I am at a loss on how to handle this issue.

      I (******************************) received a bill from Concerned Dental Care located at ******************************************. ***** ************** in the amount of $110 for dental work performed around April last year. The problem is, I was already required to pay this bill IN FULL in order to continue receiving treatment from them. I have been treated by Concerned Dental Care multiple times since the date of treatment they claim they are billing me for now. After I chose to be treated by another dentist Concerned Dental Care began sending multiple text messages and calling my phone to schedule a new appointment and they never mentioned this year-old outstanding balance. I have called several times only for them to block my attempts and I cannot get through unless I block my phone number. When I am finally successful, I am told there isn't anyone available from their billing department who can help me resolve this issue. This has occurred three times not including the numerous times I have been placed on hold for no one to ever return to the phone. I am perceiving this to be retaliatory behavior because I have chosen a different dentist. By imposing this fake bill and not allowing me the opportunity to speak with their billing department to prove it has already been paid, I am forced to perceive their actions as an attempt to push this bill into COLLECTIONS before I have the chance to prove to them that it is not valid. Please help me resolve this issue. I know it's not a lot of money, however this is an ongoing unfair practice no business should be allowed to get away with.Sincerely,****************************** ************** ***********************

      I requested a list of in network locations from healthplex dental insurance to have an emergency root canal done I went to concerned dental ************************************************************************ when i arrived I was informed they do not take healthplex and i would need to pay out of pocket after i was already given novacane I was 6 months pregnant at the time and in really bad pain. I ended up paying 2700$ out of pocket for service provided. I called healthplex to inform them i was sent to a location that was not in network by their staff however when I filed a grievance Concerned dental stated they are not in network to my insurance company but tax ID was same as being in network. when asked for a new tax ID that shows they are not in network they were unable to provide and have now been sent a check from healthplex I am seeking the refund for the 2700$ And have not gotten a response from the dental office. The service was done in april of *********************************************** Almost December.
