    My dad was here for skilled nursing and that were supposed to send him home with PT and OT and no one ever showed. I called the social worker and she said the nurse was still triaging him. She said she would call me back. No one ever called me, I can't get ahold of anyone, and he never received the therapy he was entitled to and I've had to find another one on my own.

    Okay, thanks. I'd like to respond and reopen. I don't accept because the number he gave me didn't work and when I called and left a message no one called me back.

    I am writing to report on the poor professional conduct of the Beechwood Pointe Care Facility. My mother’s doctor has relayed his concern for her health, in agreement with my previous concerns. I must note that the doctor is very frustrated with the care of all of his patients at this facility. He stated that if he could, he would pull all of his patients out. On several occasions, when I visited, I found molded food remains in clothes storage areas, dirty living areas in general and urine on the floor under my mother, on the room floor, for hours on end. I have attempted to transfer her from this facility, and I feel that they are reluctant to do so. Late September 2023 was the agreement to transfer, now there has been no effort to move forward on their part. I emailed them today and am currently awaiting a reply. If required, I have the email chain to document all. Responses from the receiving facility-we have not heard from beechwood. Response from Beechwood- we are awaiting a response from Avenue, Warrensville. In addition, I have relayed concerns during the teleconference back late September that my concerns are as mentioned above to include restrictive communications and I do not wish for my mother to be transferred in a vehicle due to her current physical condition. She has dialysis appointments. Beechwood does not offer this service in house, Avenue Warrensville does. I stated that my decision is final.

    Consumer states on Sept 18 her mother was told by the doctor her kidneys were low and was put on dialysis. She was supposed to be picked up today @ 7:30am for dialysis and has not been picked up. This could be a life & death situation.  She also states the nursing home is horrible.  They do not take care of residents personal hygiene like bathing, brushing teeth, etc.
