    On 6/25/21, I bought a membership from Fairwood. I paid $2898 down and then 12 interest-free payments of $241.50 per month for a total of $5796. The last payment was taken out 6/26/22. Initially after I purchased the plan due to Covid, the hours were changed and the business also closed on Friday, which was the normal day I went. Due to this, it was very hard for me to be seen in the office as the visits were usually 1.5-2 hours in time due to all the services received. With the membership, you received supplements to take with the program, weekly dietary coaching, use of the Tanita body composition analyzer to help your health coach figure out plans when you plateaued, use of the whole body vibration and exercise with oxygen therapy machines. You also spent time in the infared sauna and received whole body wraps and SMT stimulation each week. My plan also included several lipo light treatments that were to be initiated at the end of the 12 week period of my diet plan that I never received. Along with the initial purchase, I received several supplements to take with the diet, but not sure if those were all the supplements for the plan or if they owed me more. I called back in June and left several messages at 419-517-1030 and got no answer. I then called the chiropractic business which is also owned by the same person. i was told the business had been closed for months. She checked my acct and saw I had several unused services. Someone was to contact me for a refund now 3 weeks ago that would be in the chiropractic office on Wednesday. I have left 3 additional messages at (419) 843-1515, which is the number for the chiropractic office, and still have not received a call back. I was seen in the office but have several unused services and they owe me a sizable amount of money. Also, it is very dishonest that the business is closed and they continued to take money knowing per their website that the business is now permanently closed.

    In August, 2021 I paid $4500.00 to Fairwood for 12 sessions in a 12 week program to help me lose weight. The amount I paid also included an additional 12 sessions to be completed by August, 2022 for a maintenance program. I completed the 12 week program. I then attended 4 or 5 of the maintenance sessions when it was revealed to me that the nutritionist had quit, along with other trusted staff members. Suddenly, there was no nutritionist, which was part of the program. I was promised that I would meet with the nutritionist at each session. Today, June 8th, I had a scheduled appointment and received a phone call this morning from Fairwood, cancelling today's appointment. Counting today, I had 2 appointments left. I stopped by Fairwood today to pick up vitamins that I had ordered, only to find a Fairwood employee removing the sign from above the door. I came home and checked Fairwood's website only to find that it is permanently closed. No one called to tell me this and obviously, I will not be receiving my 2 final sessions, which I already paid for. (If needed, I can supply receipts. I cannot upload them on my computer.)

    I am responding on behalf of Fairwood Health and Body Transition in regards to complaint #********.

    Please be advised that this business closed on March 31, 2022. All current patients were advised of this when coming in for services. It was our intention to allow patient to complete services remaining within a reasonable time frame. Many have canceled or not shown for services which makes our position difficult given our staff consists of 1 person now. Another business has now leased the space making it impossible to continue seeing patients. I recently sent out letters and refunds to patients with remaining services. The complainant was one of those refunds. I can provide the letter and check if required.

    I am rejecting this response because: I was NEVER informed that this business was closing on March 31st, I continued to go to appointments thinking I had until August of this year to complete my Maintenance program sessions. I was told I could come in once a week or once a month, as long as I completed by August, 2022. There were at least 2 sessions that were cancelled by Fairwood, one was due to an impending storm, the other due to inadequate staffing. I too had to cancel a couple of sessions due to not being able to get there on time for my appointments. At the time of my calls, no one ever informed me of the business closing or that I needed to hurry in and complete my sessions.

    In response to my complaint, Fairwood claims to have sent me a letter and a refund. As of today, I have not received either. Kindly send the original letter and refund. Thank you.
