    Good Day,I received services at the hospital which included a battery of tests. I have tried multiple time to get a copy of the tests and have not been successful. I have left several messages for several departments with a return phone number and have yet to receive a call back. On 1/18/23 I called the administrator and left a message, but no response 24 hours later. I left another message on 1/19/23 and no response.The main issue I is that I delayed paying the bill unitl I could get these results. I received a bill a couple days ago and it stated that if it wasn't paid immediatley, I would be sent to collections. I paid the bill yesterday and now I feel like there's no chance of getting my test results. And the staff at the hospital do not return calls left on voicemail. Can you please help me to get my test results?

    Good afternoon.

    Our Electronic Health ****************** is currently attempting to make contact with Mr. *********************** to rectify this complaint. In reference to ****************** attestation that he has attempted to contact Lake Health District in regards to this matter, he is correct. However, Similar to the attached Release of Information request for that was received with this complaint, ************** failed to identify himself by his legally given name (***********************) he instead identified himself as "****** ***", as he does in the Release of information request (Which is subsequently unsigned). We had attempted to look for ****************** records under the last name of ***, each time he had contacted us up to this point. We could find no patient record under the name ****** *** within our system. Due to HIPAA requirements, we can neither release a record over the phone, or to any individual without first verifying the identity of the requestor and/or their ability to receive the requested patients records. This typically requires the following conditions; legal name of the patient whos record is being requested, verification of relevant personal identifiable information of the patients record being requested (email and/or mailing address on file), and/or presentation of a valid government issued picture identification. In ****************** case, we could not confirm any of this data as he did not supply the correct information or present himself in person to retrieve the record.

    Again, we are reaching out to ************** to rectify this matter, and will advise him again of the requirements to obtain records.
