    In June 2016 I received care at Cedar Crest Emergicenter. It was a worker's compensation claim that was approved by my employer, ********* ****** ********. I never received any additional outreach, communication, or bill, or anything else from Cedar Crest Emergicenter. In July 2022, over six years later, I now have a collection appearing on my credit report for $115 from ***** due to an outstanding balance with Cedar Crest Emergicenter. I tried to call Cedar Crest Emergicenter and they appear to be permanently closed. I called Remex and they are unable to provide me with an original statement. They only have their records which indicate that they picked up the collection from Cedar Crest Emergicenter last year. I want this removed from my credit as I was never notified of the outstanding balance, which is now six years old, and no one has been able to provide me with a statement for the services.
