    I was a patient at Lasting Smiles of Bethlehem until moving in 2022 and started the ****** clear aligners program in 2021. Per my conversations with the ****** company today, 7/13/2023, the date my case was created with their lab was 06/15/2021. The ****** company said my program was considered the "light" treatment program which included 20 upper and lower aligners. Per my last conversation with Lasting Smiles of Bethlehem, I received up to retainer #9 prior to moving. I requested medical records from Lasting Smiles on 07/06/2023 and did not receive my record until 07/12/2023 which included a set of x-rays only. Via email, when I asked why I have not received any of the other documents or records pertaining to the ****** program I was "Yes That is through ****** that info would come from them as they have the records." Per my conversation with ****** today, they said they do not have any records and the records should come from my previous dentist's office. I have been trying to work with Lasting Smiles for several months (2022-2023). At this point, I am requesting a refund of the money that was paid up front to Lasting Smiles.
