    This facility is filthy. Upon arriving at the entrance we were not allowed in without a mask. We called days in advance to find out what the visitation rules were and no one ever mentioned that masks were required. I advised the woman at the entrance that we had just driven two hours to get there and she advised that they did not offer masks to the public. Luckily we were able to find masks in our car. Once we entered I saw at least four staff members not wearing a mask and others with their masks not covering their face. I questioned this and the nurse advised me that because we are outsiders we are required and they are not, which violates CMS and CDC guidelines, and that she just works there. As I stepped in to my afamily members room it was in complete disarray. The sheets on her bed had tears and looked like they haven't been changed in days. The bed was not even made. They had thrown the hand-me-down blankets at the end of the bed as if she were able to make her own bed. Mind you she has a newly amputated leg, as well as, Stage 5 CKD, and Alzheimer's. There was dirt and garbage all over her floors. She was starving when we got there and it was 9:45am. I had brought her something to eat and when I went to the nursing station to ask someone to heat it up there was no one in sight. I found someone cleaning a room and asked where I could heat the food up and they directed me to the cafeteria but I did not want to enter and disrupt the service they had going on. I was able to locate her nurse after walking around and they led me into the cafeteria kitchen to the microwave. Once I entered the kitchen I noticed that the kitchen staff did not have on any gloves or hair nets as they were working. Once I went back to her room her nurse advised me that she was just reprimanded by the Administrative Person stating that we are not to be wandering around the facility. I would not have been wandering if there was a nurse available at there designated station.
