    After recovery and rehabilitation from back surgery due to a fall, I moved my mother into an assisted living apartment at Myrtle Beach Manor (MBM) on October 1, 2022. At that time her medical team agreed that she could live independently with some assistance. This decision was made primarily because she did not have any family members living close to help her, and until a more permanent housing solution could be discussed with the family. The cost of this "apartment" was quoted to me at $4000 a month plus a one time move-in fee of $3000. As with most ******** providers assisted living residences are not a covered service and she would be considered self-pay. After only spending 2 nights there my mother fell, and continued to fall over the next few days. She was transported to the hospital on 2 occasions. At this point, the attending physician at MBM made the decision that she required skilled nursing care and would immediately be moved from the assisted living wing of the facility to the skilled nursing wing. Her admission date to the skilled nursing wing was October 12, 2022. The cost of skilled nursing care at this facility was considerably more. We were reimbursed for the cost of the assisted living quarters for the remainder of October and charged the skilled nursing care daily rate for the remainder of October. The cost for care the months of November, December, and January increased dramatically with a daily rate initially charged at $180 per day for October until we removed her from their care on January 6th 2023 at which time we were being charged and paying $320 per day. Skilled nursing care is a covered service of her ******** plan, Humana, but at that time this facility did not accept Humana. We were told by Humana that her plan covered skilled nursing care regardless of whether the facility was in network and that we could file a claim for reimbursement. We contacted MBM for copies of invoices and charges related to her care for the time she was there, and after many attempts they finally provided some of the information we requested. What they have refused to provide and that Humana and all insurance companies require to pay claims is the *** codes. They do not return calls, or email requests. A Humana claims representative spoke with someone at this facility during a 3-way call with us and was told by a staff member that there were no codes in her chart. These are codes that are required by all insurance companies. They do now accept Humana and have to provide these codes in order to be paid by ******** so they are aware of what we need an why we need it, but refuse to provide it. I believe they do not want Humana to "see" and have a record of what they were charging us since we were self-pay.
