      On September 22, 2023, I had to go to MUSC in ******, South Carolina for an emergency. While at the emergency room, they had done some test and I had a scan done. A few days later I received my bill from the hospital. I had no idea that the scan would be a separate bill, and I had no knowledge that the bill was out there. I never received a paper bill, email, or phone call regarding this bill. Not even a few weeks after I went, they had sent it to collections. Since I had just turned 18, I did not have any credit. Over the past almost 2 years, I have worked so hard to build my credit. It wasn't brought to my attention that I had collections until I applied for a loan to buy land. I was denied. I called and asked if they could help resolve this issue and they said the only way to get it off of my credit was to pay the bill. After doing some research, I see that they do this to a lot of people. I would just like for the collection to be removed from my credit. I did not know that I was in debt to this company. I didn't even go there. There was no phone calls, emails, or paper bills to let me know that I owed this money.

      Got a text from this business to pay bill despite never having used their services. Many reviews on ****** seem to be that same with others saying this is a popular scam, and that hits to ones credit score soon follow.

      I had to have a ***. I called my insurance company first and was told that Ortho SC is in network and i had to pay a $200.00 co pay and I can have the *** at Ortho SC. I went to Ortho SC and had the *** and paid the $200.00 co pay. I now have a bill from Carolina Radiology for $30.86. I called them to find out why since i had paid what my insurance company told me i needed to pay. I was told by Carolina Radiology that they are in network but the doctor they selected to use to read the report is not in network and it is my fault and i have to pay. I did not know that their doctor was not in network and had no way of knowing this. I feel that it was their responsibility to check with my network to find out if their traveling doctor is in my network before they used him. I did not select this doctor, they did. I have no way of knowing what doctor they use and do not see this as my responsibility to pay. i have paid what i was told that i owe.

      I was notified by a credit monitoring service that I had a change to my credit score after further investigation I found this company has turned in a bill to collections. I have never been treated here or given permission to treat or bill me. Further I have never been contacted in any way regarding this bill. I have tried contacting someone multiple times and have not been given an avenue to resolve this. This has impacted my credit score immensely and cost me thousands of dollars. What this company is doing is criminal After Further investigation the this is a standard practice impacting multiple people and there scores

      Carolina Radiology Associates has turned an account with my name over to a collection agency ****************************. The amount is $306, date of first 3/27/2019 and last reported to the credit bureaus January 2023. This is not mine and I expect deletion from all credit files per FCRA in a timely manner.. I have attached proof of my ID, screenshot of credit report and copy of my ID theft report. This is causing me harm as it is damaging my credit rating and reputation.

      On 2.24.2019, *************************** was seen in the ** at ************************************ due to having chest pains. During this visit, he had a Chest X-ray and Stress Test performed in which Carolina Radiology Associates performed the professional interpretation of these exams. The fee for the reading of the Chest X-ray was $44 and the fee for the reading of the Stress Test was $262 which equates to the $306 ****************** is being billed from Carolina Radiology Associates. At the time of service, it appears ****************** did not have any insurance information to file so he was billed as self-pay. We mailed a statement to the patient on 3.4.2019 to address ***************************************************** (see attached) which is the same address ********************* had on file as well as the address that is listed on the patient's driver's license that he supplied to Grand Strand Hosptial at the time of service. We did not receive a response from the statement we mailed so his account was turned over to **************************** on 4.15.2019.

      In December 2021, I had an annual mammogram at ****** Health in ******** **. Although I provided correct insurance multiple times, ****** billed incorrect insurance and I assume passed that incorrect info to Carolina Radiology. In January 2022 when I recvd a bill, I contacted ****** to straighten it out. I then recvd bills from Carolina Radiology, and contacted their office to clear the matter. In March 2022, I received collection notices from Carolina Radiology and again contacted their office via web message and phone. They verified insurance and advised it would be taken care of. In May 2022, I again received collection notices for Carolina Radiology. I yet again contacted their office via phone as well as emailed the *** showing nothing was owed to a 'manager'. I was advised the issue would be resolved. I received notification now that these "outstanding" balances have been reported as debt collection against my credit. I have contacted Carolina Radiology YET AGAIN to resolve, as well as provided the *** and copies of emails to all 3 credit bureaus to get this grossly erroneous collection debt off my record. I want this resolved, I want written evidence that it was erroneous and has been removed from my credit, and I want an apology for the REPEATED mistakes made by Carolina Radiology.

      We were not notified of this patient's complaint/issue with our customer service center until *******. The same day we were made aware of this patient's complaint/issue (12.9.22), I immediately had **************** charges closed/cancelled with the collection agency and asked the collection agency to send a written notification to her via mail stating that her balance was closed and will no longer affect her credit (which they confirmed they did so on 12.9.22). I then had one of my representatives reach out to the patient at ************ and it went to voicemail where my representative left a message for the patient to return her phone call so she could let her know that this has been resolved with the collection agency. The patient never returned my representatives phone call.

      I am attaching the e-mail trail showing when I was notified of this complaint (12.9.22 sent to my e-mail address of ********************* and showing where ******************* with ************** Agency confirmed the cancellation/deletion of this patient's charges along with ************** Agency's written letter that was sent to the patient on 12.9.22.

      This account has been closed with ************** Agency and is resolved in our billing system as well.

      I visited McLeod Seacoast Loris hospital in January of 2020 due to a broken collar bone from a motorcycle accident. McLeod Loris uses Carolina Radiology as an X-ray affiliate for billing or some other sort. I presented the ladies at the counter with my insurance info and they ACCEPTED MY CO-PAY for my insurance. However, it seems the ball was dropped by someone and now I'm paying the price because my bill has been turned over to collections. My insurance can confirm there has NEVER been an attempt to contact them and I've done everything o was supposed to do. This needs to be removed from my credit report as I was not the one who caused this problem. Attached is my insurance info.

      Business Response /* (1000, 8, 2022/05/19) */ We received this patient's information in our system on 1/30/2020 via an electronic download from the facility where the patient was seen (McLeod Health Loris) - The demographic information provided by the facility stated this patient was self pay (no insurance provided) - initially an e-mail statement was sent to the patient at e-mail address ****************@GMAIL.COM - we gave 5 days for the patient to respond to the e-mailed statement - once no response was received, we then mailed a statement on 2/4/2020 to the patient address provided by the facility at **** ******* ***** *** ****** ** XXXXX-XXXX. No response was received from the patient on the mailed statement so it was turned over to SCA Collection Agency on 3/17/2020. SCA Collection Agency then mailed 4 collection letters (one letter for each charge) to the same patient address - no response received so the account rolled to bad debt collections on 5/18/2020 - the collection agency then rebilled all charges to the patient again with no response - The collection agency rep then tried to reach the patient via phone on 12/10/2020 at 843.340.7618 with no answer and no return call from the patient. The first time the patient reached out to our office or the collection agency for the charges incurred on 1/25/2020 date of service was on 5/9/22. Based on the above details, there were several attempts made by Carolina Radiology's billing company and the Collection Agency to reach the patient to either provide insurance information to file or to make payment on these charges with no success. I have attached a copy of the statement we mailed to the patient for your review.
