    05/19/2023 services were $6200.00 this amount was paid . 1400.00 was paid at the consultation the remainder of the 6200$ was paid at the first appointment. The 6200$ was to include the extractions of teeth, the molding to get a set of temp dentures as well as the perm dentures. two bones were also supposed to be removed from the bottom and top gum.that the dentist had left after the extraction. at the time of the first appointment the office would not provide me with a receipt or treatment plan showing what was done or what was to be included.it was all discuss in the office with the dentist. we go to the first appointment and have the extractions done and the temp moldings i leave the office with my mouth full of blood. running down the sides of mouth.i was supposed to go back in two weeks to have an alignment done to get my dentures to fit . they could not do the alignment because the mouth was still too swollen and the bone needed to be removed from the gum. three weeks after this appt i went back to have the alignment done. at the end of the appt i went to the receptionist to schedule the next appt which the dr said to schedule 6 months from now this appointment was to get the prem set of dentures. at this time the receptionist told me there was no more appointments that if i wanted the perm set of dentures or to have the bones removed i would have to pay extra for that. when i was informed it would be included in the 6200$ i called that day later and i requested a copy of the treatment plan that day she agree to send me a copy of the treatment plan . which she never sent it. i called the next day and requested a copy of the treatment plan again she said it was not done yet. three hours later the office manager called me and said this was not in their system that the 6200 included only the extractions and temp dentures. at this point i would like a refund i feel i was treated poorly and taken advantage of because im hispanic and do not speak good english.

    A treatment plan with procedures and costs was provided to the patient who signed it and agreed to it. All procedures agreed to were performed and the patient has received copies of all signed documents and receipts. The patient had his wife as a translator at every appointment.  We can not elaborate further without violating the patient's private health information.

    The patient has been emailed to the address they provided our office. The patient's wife stated on the phone on 6/21/23 that she had the forms he had signed but could not locate them at the time.  She was sent copies again to ************************ which was the address provided to the office as a way of contact. These copies were sent again on 6/29/2023.

    I am rejecting this response because: i received the email and everything on the email is not what I had discussed with the dental office . I want to see the actual treatment plan where I signed what I was getting done and how much . Because this is not what was agreed upon.  All this is is a breakdown of prices and none of it adds up to what I was told .
