On October 26 2024 I was injured on the job. My ********* comp department ordered me to report to Concentra Urgent Care for treatment. I was seen by several physician's in this establishment. The treatment plan that they recommended was backwards which was gross negligence on their part. They had me start physical therapy first which ran about 2 months. I had to report to them at Concentra bi weekly to report and see how PT was going. So the last visit with ****** ****** PA-C was very uncomfortable because she asked if you're not getting any better. I said no, and her response was very subpar and dismissive, not what you expect from a healthcare provider that has taken an oath. She tells me we will send you for an *** to make sure we didn't miss anything. I'm saying to myself that should have been the first thing you all did if you wanted to treat me correctly for my injury. I didn't rebuttal or act combative. On January 1, 2025 I reported to the imaging clinic to have the *** taken. I went back to Concentra to get the results of the *** of my left hip area. ******* ****** was who I saw on 01/21/2025 he told me they found torn tissue in my Hip area there was nothing else that they could do for me. I said I'm still in pain. Why would you release me today and I'm not properly headed? This was a slap in the face which caused me to become aggravated and send me into a black hole of depression. I'm just getting myself together mentally to expose the blatant negligence and slap in the face and he was forced with his decision. I limped out of that office so fast so I could cry my heart out in private once I got to my car. It is almost like watching your mom fall down a flight of stairs and you step right over her to get your phone to check your email. Imagine that!Initial Complaint

I recently received a job offer for janitorial position in which I need to get a physical exam to make sure that I can do thejob I came to the doctors office in ********* to get my exam done and I disclosed all the information regarding any injuries or disabilities that I may have .The physician that I see will not let me take the lift test until I get a clearance from my doctor so I went to my doctor on March 10 And go to the valuation and cleared the next day. I came in to give them the clearance note and Concentra said I needed to fill out form from my doctor so I went back to my doctor and they told me they do not fill out forms that I just need togive them my medical information and hand it to them attached to the form and concent will take it. I came back today on March 12th because the physician at Concentra was off on Tuesday the 11th that is when I found out that a simple clearance from my doctor was not sufficient soI went back to my doctor to get the information and they told me to give my medical records of my condition to the doctor and should be sufficient enough. I came back today on March 12 to get the remaining part of my exam done and they will not take information telling me that ****** will fill out the paperwork, but already spoke to them and they will not fill out the paperwork so now Im left in limbo because I cant get my test done becauseno one will help me and this physician that I am seeing here at Concentra I have dealt with him a few years ago before and it was the same problem. I almost was paralyzedbecause he didnt wanna do anything for me unless I went to get a MRI at my doctor, thank God I got an MRI for my doctor because if I wouldve waited longer, I probably would not be here todayInitial Complaint

I took a drug test to return to work and after almost 2 weeks I still don't have the results and can't work. Evertime I call I am put on hold. I have made 5 calls last 2 days and never have been picked up off hold. I need these results to work!Business Response

Importantly, Concentra was only a collection site for this drug screen and as a result, Concentra did not receive the results for this collection. This was a Labcorp specimen and we recommend that you reach out to ******* directly for the results. Hello,Im experiencing some challenges at the concentra in ***************, ********. I previously visited for a DOT exam to qualify as a bus driver in the *** metropolitan area. During the exam, everything went smoothly, and ***************** was the physician overseeing my assessment. I was transparent with her, as I believe honesty is the best approach. I successfully passed all of my physical tests, and I have the documentation to prove it. This occurred on January 20th of this year. Dr. ******* ****** informed me that the last two forms need to be signed and returned or faxed to Concentra in order to receive my DOT. I faxed the forms to them, and they were mailed to me. I received them yesterday . I dont have a fax so I had Ms.**** ******* fax them for me after meeting with her. I visited Concentra today in hopes of speaking with ******************* but I found out that she no longer works there. I questioned her email and reached out, but have not received a response. Additionally, when I searched her name online, I only found negative reviews. This has been disheartening, especially since I'm trying to turn my life around and establish a career to support my two children.I inquired with the front desk staff about speaking to any practicing doctors, and I was told that the doctor *** conducted my physical is the only one with my records, a fact I was previously unaware of. I asked when the doctor would be available, and I was informed she is out for the remainder of the month, with no clear return date. Ive been calling Concentra since January 24th, and they consistently verify my name and date of birth during the calls. After that, they inform me that the doctor is with a patient and that both the doctor and staff have my contact information and will get back to me. However, they have yet to do so even once. Today, being February 4th, I decided to drive there since I had some free time.Business Response

The patient presented to Concentra for a safety sensitive **************************** (DOT) examination. During the examination, the patient make a disclosure and as result of said disclosure, was medically not certified to operate a commercial motor vehicle. In response, at the time of the encounter, the patient was provided paperwork to bring to a professional to facilitate an evaluation related to the disclosure.
The patient subsequently provided results of that examination. Now that the patient has the examination complete, the patient is eligible for re-examination. The patients employer was notified that he has returned with the appropriate documents. Concentra is now waiting on the patients employer to request and authorize a re-evaluation for pre-employment. Upon review of your records and after consulting with the Concentra clinician who saw you for your visit, your DOT card was denied due to a disclosure that was made during the exam. The disclosure disqualified you for a DOT card and the reasoning behind the disqualification was explained by the Concentra clinician. It was also explained that should the disclosure change, you would be able to obtain a DOT in a reasonably short amount of time.

I am writing to formally file a complaint regarding the negligent medical care I received at Concentra following a car accident that occurred on 8/1/2024. After the accident, I sought treatment at Concentra due to persistent pain and discomfort most recently on 11/5/2024. Ill However, despite multiple complaints about ongoing pain, my concerns were not adequately addressed, resulting in a worsening of my condition.Throughout my treatment at Concentra, I was only seen by a Physician Assistant (**), who provided general care but did not address my recurring pain with the thoroughness or escalation it required. I expressed my concerns about persistent pain on several occasions, hoping to be referred to a specialist for further evaluation. Unfortunately, no referral was made, and the care provided did not include the necessary steps to fully assess my *********** a result of the lack of appropriate follow-up and failure to refer me to a specialist, my injury has deteriorated over time. Recently, I was compelled to visit the hospital, where I was informed that surgery may be required due to the prolonged effects of an untreated injury. Had Concentra taken the proper steps to refer me to a specialist or conduct a more in-depth evaluation when I initially reported my symptoms, this situation may have been avoided.I am deeply concerned about the potential impact of this negligence on my long-term health and well-being. I urge Concentra to review this case carefully and take responsibility for the substandard care I received. Furthermore, I request information on how Concentra plans to prevent similar incidents in the future, so that other patients are not subjected to inadequate care or unnecessary health risks.Please let me know the next steps in addressing this complaint. I went to Concentra on **** ******* blvd in **************** for a work injury, right ankle sprain. Once cleared, I was seeking new employment. My new employer told me I needed my release paperwork to specify that my clearance included driving commercial vehicles, which is my trade and career. I went to get this addendum on my records and was told this was not allowed and that I had to have an entire new appointment to get the paperwork stating this. When I showed for the appointment, which was paid out of pocket, I was told it was the wrong type of appointment and I had to do a new physical. I went along with it, trusting it would get me the paperwork I needed. At the end, the PA gave me a new medical card and when I asked for a note specifying commercial driveling I was informed that it would be illegal for her to do this and that my med card should be enough, even after I was told and told her that it was not. Per the new employers insurance, I needed paperwork with the clarification that she was refusing to give to me while her and the front desk staff were all condescending and treating me like a child instead of an adult who is trying to get a job. I am currently unable to find employment in my field without this note and she continues to refuse to provide any further assistance.Initial Complaint

I went to a concentra urgent care to do a drug screening for my employer. The concentra is on *********** in ****************. There was a black guy there who i went to school with that never liked me so he basically was throwing my paperwork at me and constantly getting smart with me. Then he had a white lady to come remove me from the room and make me leave without talking my drug test or my physical and they banned me from all concentra locations where i cant go to any in ******** anymore but i have a job offer and my job only sends people to concentra but they wont let me come back there. On the day of July 24, 2024 on Wednesday at 11:05 AM I went to Concentra on 10th in Shadeland for a drug screen for velvet ice cream I gave the information to the receptionist for velvet ice cream dress green after they call my name went back to the back and did the screen a younger guy who didnt ask me if the information was correct or anything he sent my personal information to ************************ instead of velvet ice cream. I never apply For ************.
