  1. On July 17, 2023, $4,800 was paid to ComfortHome Caregivers LLC by Robert ********. Note new address: 1712 North Frazier Street, Suite 213, Conroe TX, 77301, Phone: 936-306-5691. 2. ComfortHome Caregivers was to provide 33 hours per week of Home Health Care services for Robert ******** over a 60-day period beginning July 17, 2023. (See attached fee estimate and contract). 3. Robert ******** had a fall on July 26, 2023 and was admitted to Memorial Hermann Hospital that day. He remained in the hospital until he was transferred to ************ ******* ******* ********, Conroe Texas on August 2, 2023. At that time, we informed ComfortHome that their services were on hold effective August 2, 2023. (see attached email and response). 4. The dispute is that we maintain that ComfortHome only provided 10 days of home health care services at 33 hours per week or 60-70 hours total during the period from July 17-July 26, 2023. However, they were under contract to provide 60 days (8 weeks) of services at 33 hours per week or 264 hours of services. They refuse to refund any of the funds for services not provided. We have written them a letter requesting a refund for services not rendered (see attached letter) during the 60-day contract period. They have written a reply stating that all of the funds were spent. (see attached letter). 5. We request a refund of approximately $3,400 for services not rendered by ComfortHome. 6.
