Two separate Refund Checks from Integrity Urgent care were sent to *************** of ************************************************************************************* or phone contact. Once opened the checks, both checks had two separate serial numbers. In attempt to cash both checks at local bank, ******************* ***** on 06/04/2022. Both checks were cashed with no issue or any notification of a false check or voided check. 5 days later on 06/09/2022, I checked my bank account at ******************* *****, a negative impact of my bank account of the account $-102.28. Within an hour the contact was made to Integrity Urgent Care in regards to the issue above, Spoke with Management by the name of ******* who asked me to provide bank statements which were sent via Email. Once email has been sent ******* then assured me this issue will be sent to upper management of the account department. Also inform *******, ******************* ***** will continue to make charges on my account until the negative amount will be paid off. ******* than voiced an understanding in regards to this statement and later stated continue to send an email of the charges made on the account, I voiced an understanding. 06/09-06/15 I have been in contact with Integrity Urgent Care and *******, to inform me no process has been made and still pending a review on the issue at hand. Today, 06/15/2022, ******************* ***** charged my bank account $34.00 due to account still in a negative affect, with leaving my account with a total $-136.28. Again in contact with Integrity Urgent Care and spoke with Supervisor ***** in regards to the issue at hand & stated We, Integrity Urgent Care are not responsible for the charges made on the account, as I was not entitled to the second check. However prior to both checks being sent out to my home address, no contact has been made to make me aware of the second check being void or sent be mistake nor was ******************* *****, due to the charges that were made on my account
