    On September 16, 2024, I met with Dr. ****** ***** at Northeast Dallas Dentistry. Initially, I was told Dr. ****** was a contractor and not employed by Northeast Dallas Dentistry. However, her billing would be IN network with ***** HMO for the periodontal cleaning. I was billed $1276.00 for services from Northeast Dallas Dentistry. NOTE attached paid ********** concerns are listed below.1.Why was I given an invoice with HMO pricing for procedures that ***** denied?2.Why was I given an invoice with *** pricing that was greater than ***** contracted pricing?3.Why are anti-bacterial procedures (D4999, D4381, D1330) not covered for a periodontal cleaning? 4.Why was ******* (D4381) denied? NOTE TREATMENT AT NORTHEAST DALLAS DENTISTRY Detailed Account Information shows coverage and pricing for ******* D4381. 5.Why does D4999 - BAC Decon/Qd show on my invoice, and D4999 - UNSPEC PERIO/BY REPORT on the ***** claim?NOTE TREATMENT AT NORTHEAST DALLAS DENTISTRY Detailed Account Information shows coverage and pricing for BAC DCON/QD D4999. 6.Why does D4341 Scaling and Root Planing, 4+ Teeth - $139.00 appear on invoice, and appear for $115.00 on ***** claim?7.Why does D1330 - ORAL HYGIENE INSTRUCTIONS appear on TREATMENT AT NORTHEAST DALLAS DENTISTRY - $114.00 and the procedure was denied by *****?The price columns (UCF, Estimated Fee, Estimated Patient) do not reflect my actual out-of-pocket costs. The invoice is misleading and confusing. Northeast Dallas Dentistry knew or should have known the procedures that would be covered, denied, and those with pricing changes or differences from the ***** contracted pricing. I was handed an estimate/ invoice and seemingly expected know this information. Coverages and pricing were not discussed with me prior to cleaning. This was unfair and less than transparent. Northeast Dallas Dentistry knew I was being overcharge. These actions and inactions amount to unfair billing practices.
