      I received a bill this month for both ****** ***** ****, and ***** **** and I tried to call the phone number listed on the invoice and it continuously said it wouldn't work that the party was unavailable and it had a ******* message at the end. After multiple attempts, I finally got the correct phone number to the local place and they told me that phone number on the invoice was the only number that existed that they were aware of. He told me that whenever the number didn't work they had to use an email and she provided me with one, the email address was billing at *********************** of which my emails got rejected. I am trying to find out what a date of services what this is for and why there's notes at the bottom of these that say that they have gone to collections when I just now received, I would appreciate someone to contact me via email to figure this out.

      Statement Date: 05/13/2024, Invoice # ****, Account Number ******************************************************************************** will not provide me or Tricare4Life, my secondary insurance, a copy of the UB04 which is needed by Tricare4Life so that they can proceed with paying off their part of the bill.Further explanation is stated on the Explanation block below. I did not attach a copy of the bill due to HIPPA, however, I am willing to provide you a copy if required.

      I have requested my health records and itemized bill since October 24, 2022 at least 3 times through the submission website for both records and billing, and countless others via email. its been over 3 months, and I have not received my records or billing.

      I walked into the *********** Health clinic on 12-19-2022 presented with a fever and explicitly reported sinus infection symptoms after a dental procedure. Several weeks later my insurance carrier and myself are being billed for duplicate services, duplicated CPT codes under different claim submissions and for services I did not ask for or need or even told were being conducted.Claim: ********* in the amount of $12114.00 included the following codes/procedures in question:CPT Code 99284 which is designed to distinguish the level of medical decision making required for this visit."The provider sees a patient for an emergency department visit involving evaluation and management (E/M). The visit involves a moderate level of medical decision making."Then on the same claim, I was billed twice for the same Influenza Assay with Optic CPT code: 87804 each in the amount of $575.00Additionally on the same claim, I was billed for services not needed, not asked for and/or not advised were being peformed:CPT code 87880 Strep A Assay with Optic in the amount of $1038.00CPT code 87426 Sarscov Coronavirus in the amount of $360.00not to mention some exorbitant charges for a single pair of gloves and alcohol in the amounts of:$50.00 & $276.00 respectivelyThen we move onto the 2nd claim # *********** submitted for duplication of "Emergency Services" using two different CPT codes again where only 1 CPT code per visit is expected since again this is meant to determine the level of medical decision making required for this visit presented as "sinus infection" which I could personally consider neither code in this instance but both CPT codes were submitted under two different claims:Claim ********* *** code XXXXXClaim ********* CPT code XXXXX for the exact same visit on 12-19-2022These fraudulent and exhorbinant charges must be corrected and eliminated where duplicated and/or attempt to recover twice. This is exactly the example why our global health crisis is in the state it is because these types of crimes are allowed to go unchecked.
