Details of my complaint:I received a notification from ********************* on December 20, 2023 that I owed ******* for *******s provided from Singleton Associates, P.A. I called CMLP to find out details of these *******s and they provided me with date of ******* of ******** 29, 2022 and patient date of birth ******** 30, ****. This is not my date of birth, nor did I receive any care of any kind on the date provided. I disputed the claim and am still receiving notification that I must provide documentation to prove I DID NOT receive *******s. I am also being threatened that if I do not remit the desired payment, it will go on my credit report. I feel that proof of the claim should be provided by the institution requesting the payment rather than the individual who is being falsely accused of receiving *******s.I feel that this is harassment and fraud and would like these notifications to be stopped immediately and my credit report to be returned to good standing.I have also reached out to Singleton through their website with no response.Customer Answer

On 12/30/2022 at 12:51 pm from phone # ************* my daughter received a phone call asking for my husband. She was then asked if she was ********************* (which she is) and then asked if she lived at our address, which she does. The debt collector then started discussing the debt with my daughter. The representative from this company broke collection laws and verification laws. The representative also argued with her about the situation when she was trying to hang up the phone. Very unprofessional and I am looking for a representative of this company to reach out to me as soon as possible to rectify this.Business Response

Received a bill for services never provided. I have never been to Houston or been treated by or at singleton. **************. I need this removed. This is ridiculous to get a bill from a***** I've never even been to in the last year. This is unnecessary harassment. You guys need to get your billing together. It's not on any of my credit reports but you guys sent me a bill. I will be filing this to the BBB and reporting your shady practices. This company needs to be shut down for extortion. You guys sent it to an address I haven't live in at over 3 years. So it makes me wonder how you even obtained that information illegitimately. I don't want to hear that it's what you have on file. Because I haven't used that addresss in years. I want to speak to a real person from your company. I want a name and phone number. If this does end up affecting my credit I will be pursuing this a lot further. This company seems like a scam and they do this to a lot of people because they don't want their credit affected. So give me a name and phone number. I've never been to Houston or gotten treatment in Houston. So I want to know how I'm getting a bill for services I've never received.Business Response

I received a collections notice on 12/6 saying I owe $140 to Singleton Associates. I've never been to Singleton Associates nor do I live anywhere near them. They are fraudulently reporting my information to debt collectors and now I'm having to deal with that hitting my credit report and filing a dispute.Business Response

I received a letter from a debt collector ******************** to try and collect 584.00 stating services were provided by a company called Singleton Associates P.A. in Houston TX. I have never been to Houston for any medical or doctor visits and do not live in Houston. Attached is the document I received for services that were NEVER provided by this company I have never heard of or been to. I have never had any sort or radiological or lab work done in Houston. I have also confirmed thru my insurance provider no claims have been sent to this company. Research indicates this is a scam and do not want it to appear on my credit report. I need documentation from both ******************** and Singleton Associated P.A. acknowledging error on their part and will be corrected with no further action taken by them. I have received a notice of collection from a collection agency from a provider that I have never used. When asked for validity of the charge, I am provided with a generic post office box address. I pride myself in paying my bills and certainly would not jeopardize my credit history for a charge that is less than $100. This feels like a fraudulent attempt to gain access to my financial information. This can be easily remedied. Provide me with the details of this so called medical visit and subsequent charges. I received an invoice from a collection agency ******************* about 11/28/2022. It listed Singleton Associates PA as the party involved but no date of treatment is mentioned. For the past week I have attempted to get to the bottom of this invoice and I am so frustrated. I did have various tests done in April 2022 but my physician and hospital have not been able to provide me with any information. Although I reached out to Singleton on 11/28, they could not resolve the issue for me. They did confirm they had used an incorrect billing address for me. Apparently my insurance company denied the claim due to "course of treatment was not pre-approved" but I believe that is because it was miscoded when transmitted to the insurance company. All other bills related to this incident in April 2022 have been satisfactorily handled by insurance. This service should be covered by my insurance. Today, Singleton provided me with a copy of the invoice but told me it was up to me to file with my insurance. I am unsure of how to proceed -- if the invoice is miscoded, then it will be declined again. I need Singleton to review the invoice and refile it with my insurance company so that it can be properly accounted for but they are uncooperative.Business Response

On August 31st 2022 I went to Providence hospital in Texas. I received two bills, one from the hospital itself and the other from Singleton associates for the radiology part of my assessment. I was contacted several times by Singleton associates and each time referred them to my health care provider for claim of my bill. They contacted Kaiser Permanente my health care provider one time and the claim was denied because Singleton didn't submit the claim to the correct address. I then received a credit collections letter from credit management on behalf of singleton. I called Singleton today along with ****** of Kaiser claims department and they will not accept any information from either myself or Kaiser regarding where to submit the bill. It seems like Singleton associates is trying to scam me by sending me to collections because they can't follow the proper procedure to get paid. I cannot get in contact with Singleton without going through a barrage of questions and they are not helping me at all. I am hoping by filing this complaint that I will receive some assistance from someone in the United States.
