      My husband went in this facility to pick up a **** form that I was advised it was completed and ready for pick up. Since I had given permission for my husband to pick up forms he stopped by on November 28, 2023. Before leaving the location he texted me the images of the completed form to which I advised it was not completed as I had discussed with provider NP ******************* on previous weeks November 10, 2023 at the same location on night clinic hours. The provider **** had discussed with me that the person in charge of filling out this forms(******) will be doing so but it will be taking a few weeks and she will be calling me (the patient) to discuss accommodations needed on the **** form before submitting. The provider also stated on the discussion with me that these forms needed to be filled out pretty much selecting all boxes as I do have a few conditions that meet the criteria. Anyway, the forms were not filled out with any of the medical information the provider ******************* and I had gone over with on my in person visit with him on November 10, 2023. When my husband picked up forms on my behalf he explained to ****** the forms needed to be filled out completely and accurately to which office personnel advised they were not going to. Not only did the provider charge to fill out forms but they also neglected to act accordingly to which office manager **** stated they were not going to refund the forms payment nor was the provider ******************* going to finish the forms with my medical information as requested. I am seeking refund for forms not being filled completely and the fact that they were filled with just blank statements as Needs more evaluation from rheumatologist when that information is already on my record. Also, the main provider Dr ********************** Abalihi has no idea how this practice is being runned as I have NEVER once seen her only PA or NP to which keep getting changed every month as previous providers quit after malpractice and patients/parents complaints.

      On 09/28/21 I went to this provider and was advised they are out of network so I paid the amount of $75 for the urgent care fee. I received my explanation of benefits on 03/10/22 and saw that I actually only owed the provider $1.07. I called the billing office on 03/24/22 to request the refund since they were also paid by my health plan. $199.63. For the past 3 months I have been pushed back and forth to 3 different people to get this refund back. Spoke with ****** in their billing group, ***** the office manager and about a week and a half ago now ***********************. They have been going in circles and continue to change their excuses on why they cannot provide my refund that is rightfully owed to me. I have also submitted a complaint with my health plan against them and want to be sure this is acknowledged since they refuse to pay me back what they have already received two payments for. This seems fraudulent on their part and a horrible place to do business with. I have been patient for 3 months and now being told I will need to wait another month for more excuses judging on their professionalism so far.

      Got billed for a physical from one of my kids, it is preventive, I shouldn't be billed, called several times to get this resolved and never got a call back, I stopped receiving the bills now I get a collections letter, what a waist of time and money never going back
