    On ******, I was notified by the MRIoA that on ******, MRIoA learned that they were the victim of a sophisticated cyber-attack that resulted in the theft of protected personal information for my wife, *****, (a member on my health insurance policy) that *** have included: demographic information (first and last name, gender, home address, phone number, email address; date of birth, social security number); clinical information (medical history/diagnosis/ treatment, dates of service, lab test results, prescription information, provider name, medical account number, or anything similar in your ********************** file and/or record); and financial information (health insurance policy and group plan number, group plan provider, claim information).On ******, to fully assess the severity of the information stolen and consider actions needed to be taken, I asked MRIoA to provide: For each data point/element listed above AND for all other data points/elements that they had that are not included in the list above:a) Exact information contained in each data point/element (i.e. data point "diagnosis" = *************** b) **** that the data point/element was placed in your system (or received by MRIoA), c) Specific reason MRIoA had the data point/element (specific medical review or specific health care service being reviewed.), d) Name of entity who provided the data point/element to MRIoA.The MRIoA letter was general and lacked specific detail about exactly what personal data was stolen. My request is consistent with data security breach notification guidelines listed in ******* State Statute *******, Amended October 1, 2019, and current in 2021/22, in Section: Mandatory Notification Items - Item (3) Information that the individual can use to contact the covered entity to inquire about the breach of security and the personal information that the covered entity maintained about the individual. MRIoA HAS IGNORED ALL COMMUNICATON.I have more to discuss but not enough space.
