      7/27/2020: Dentist performed crown work done and crown was permanently glued down. Dentist told me that it should last for 9-10 years or more. However, it was not glued correctly and started to have a sensitive pain issue. I called several times to complaint, but dentist could not do much to fix the problem because it was already permanently glued down. 10/15/2021: After little over a year later, crown came off all by itself. I took care of the crown very carefully and did not chew or eat any hard, sticky, chewy food. There was no reason the crown comes off little over a year later. It may not glue down correctly initially which creates the tooth sensitive issue. I called dentist and made an appointment to reattach the crown.11/2/2021: Dentist checked the crown issue and try to find what the problem was. He suggested to put the original crown attached a month later. From 11/2 - 12/16, temporary crown was applied, and my tooth sensitive issue was gone during this time. Dentist told me that no cost to the patient for re-applying the crown. 12/16/2021: Dentist took the temporary crown off and permanently glued down for original permanent crown. However, I still have a sensitive tooth issue after attaching the permanent crown and have to live with it for now. 1/10/2022: I received a statement from my dental insurance company stating that dentist charged, $116.00, for re-cement work for the 12/16/2021 service. It ended up generate the deductible amount, $60.00 to the patient. Dentist should not claim the re-cement service performed on 12/16/21 and I should not be liable for paying $60.00 deductible to the dentist. This recement service should be free of charge to the patient due to initial crown was not glue down correctly. Dental insurance company: ***** ****** of Virginia. I submitted FRAUD complain on 2/15/2022 Dentist office ledger shows "$0.00" for re-bond crown but they submitted "$116.00" to the insurance company.

      To Whom It May Concern, 

      This claim in question was corrected with *** *****'s dental insurance on 02/14/2022. We sent a narrative to ***** explaining the error with the billing vs the ledger- we submitted a $0 procedure, but the claim format was not changed to the ***** to reflect the change, which resulted in ***** ****** receiving a claim with a dollar amount for the re-cementation in error. 

      The same day *** ***** complained about the balance, we corrected the claim with ***** ******, so we are not sure why a complaint was made when the issue was resolved/the patient has not been billed since January before the mistake was corrected.

      Attached is a copy of the narrative we sent to ***** correcting the billing error, a copy of the corrected claim from *****, as well as a statement showing a $0 balance for this patient. The problem has already been corrected and has been corrected for months. We request that the patient please verify the issue has been resolved before making a formal complaint.
