Be aware of the lack of attention to detail this doctors office has. My family has been going to sound family medicine for over 10 years. My minor child was given a vaccine without parental consent. The scary thing is that 2 different clinic staff both assumed there was a signed consent form, so neither one looked. Careless and reckless staff.

      Hello,

Per HIPAA rules and regulations, we cannot release confidential information surrounding patient care.

We are comfortable sharing with you Washington State Minor rights laws surrounding immunizations.

Feel free to go online and look this up if you would like as well, the website is: ***********************************************************************************************************

In ****************, minors may receive immunizations without parent consent under the Mature Minor Doctrine.

Minor rights is a very delicate topic with parents. We follow the Washington State rules and regulations surrounding minor rights.

We have attached a document that shows this information as well.

      Thank you,

On 01/23/24 I had a physical that was paid for by my health insurance. At the time of the visit my Dr asked me if I had any questions or concerns. I stated that I still had problems with my legs from an on going problem I have discussed with the Dr for the past year. The Dr's ****** charged me for an additional ****** visit on the same day. I refuse to pay for fraudulent charges.

We try very hard to keep our patients informed of what is covered during a physical.  Prior to patient Physical visits each patient receives a form that clearly shares what insurances cover in regard to a physical, and that should other concerns arise during the visit they *** result in additional charges. The form states the following:

      "Preventative Wellness Visits:
What Does Your **************************************** insurance companies cover a predetermined list of components that are considered part of a well child check, adult physical, or ******** annual wellness exam.

For example, during a wellness visit, you *** expect that your provider will do the following:

Gather or update your comprehensive medical history
Outline plans for the reduction of risk factors
Provide interventions or counseling to improve overall health
Order appropriate laboratory/diagnostic procedures based on your personal medical conditions
      Order and/or administer appropriate immunizations

Manage minor health concerns. A minor health concern *** include the following and other similar concerns:

Obtaining a refill for a prescription for maintenance medications (allergy or birth control, for example)
Asking your provider to pay special attention to a skin lesion.

More specific health concerns *** not be covered by your insurance as part of your wellness visit. These are concerns that would normally prompt you to schedule an appointment if you were not scheduled for your wellness visit. If time permits, some of these concerns *** be addressed at the time of your wellness visit; however, it *** be appropriate, according to your insurance guidelines, for Sound Family Medicine to generate a charge for an office visit, which would *** result in a co-pay, deductible, or co-insurance responsibility depending on your plan benefits. If time does not permit, we can schedule a separate appointment.

Following are examples of specific health concerns that *** not be covered as part of your wellness visit:

      A sinus infection
Sore throat (and testing for strep)
      New cough
      New or changed headache pattern
      Abdominal pain
      Pelvic pain
      Depression or anxiety
Joint pain, both specific and general
      General fatigue
      Sleeping problems
      Irregular periods
Significant changes in status of a chronic health problem"

If the patient signs a HIPAA agreement with SFM we will be happy to provide you with any documentation or forms.

If you would like additional information surrounding what is covered during a physical there is a great article on AAFP that discusses this in an easy to read manner. *******************************************************
