1 starI would give them a zero if possible. I had a partial completed by their office a was forced to put half down. My partial was completed and my insurance paid for a hundred percent. I am getting the run around to receive a refund. They claim it comes from their corporate office and refuse to provide contact information for their corporate. I would not recommend them unless you want them to keep your money!!!Review fromJulie G

1 starCalled Forward Dental in ********* to make an appointment for a cleaning. My (new)insurance uses them as a provider. However, the person I talked to at Forward Dental told me they no longer accept insurance from ************************************. And thenthe person on the other end tried to sell me a yearly membership for services that would deduct 10% to 15% of all services.Sent an email to the insurance company asking them why the clinic is listed as a provider but they, Forward Dental, are saying no. Insurance company assures me that Forward Dental in ********* is in the network and will reach out to them. Get an email back from insurance saying the problem is that Forward is in network but the dentists listed as being there are no longer thereand the resident dentist is not in network even though the clinic is. ?????I get the feeling that they are scamming the insurance company and patients. Most, if not all, of the dental providers listed as being in network with ************************ are Forward Dental. I will try and make an appointment at one of the other clinics and see what happens.Review fromanthony- k

a nightmare ! about two years ago i made the huge mistake of going to forward dental, river north location first. after exams, x rays, ect. the dentist said i needed false teeth, got fitted everything all ready, insurance accepted he referred me to an oral surgeon. took about two months wait for appointment. got all of my upper teeth removed. went back to forward dental - guess what? dentist quit - office closed down. now i have no teeth and no false teeth. all of that time and money wasted. on my own, i found another location at Glendale. started all over again from scratch and they also accepted my insurance. after about another four months - i finally got false teeth ! did not fit - cut into my gums. tried about three or four more times - still do not fit. also they informed me that my insurance did not pay them, so i paid them one thousand dollars, as they said they will reimburse me when paid. another six months i gave up - still cant wear teeth that do not fit right. going somewhere else. today i get another bill for another thousand dollars - said my insurance is not valid ! not true - my insurance is good. i am hiring an attorney and will start my law suit immed, as well as reporting them to the proper authorities. Stay away from these scammers - DO NOT GO TO FORWARD DENTAL........anthony- k

update - contacted my insurance company , that forward dental accepted ran my card, and said no problem - than now say i owe them another thousand dollars because my insurance didnt pay. guess what - they never tried to bill my insurance company at all - just forward dental lied to my face. that is the **** to get you into the clinic - than chase you for ALL of the money afterwords. seeking legal help now, and filing court papers against them. do NOT trust them...........Review fromJeff B.

1 starHad been going to Forward Dental since ****. Had a few billing issues but worked them out. But the last three years have had nothing but issues. Never had the same dentist or hygienist while going there. We were asked to move to the developmental drive location that was just opened and would have a steady dentist working there. This was untrue. Over the past year had to have a tooth pulled because the dentist stated it had an abscess next to the tooth. Had it scheduled and a day before the appointment, the office person called and wanted to know about my implant that I was scheduled for. I was never scheduled for that and I told them I was coming for a tooth extraction. There was never a discussion for an implant. When I arrived for the tooth extraction I asked to confirm if it was necessary to have it pulled. The dentist that was working said yes and started to numb the wrong side of my mouth. I asked what he was doing that the tooth was on the other side, he laughed and said he looked at the wrong information. recently an appointment was canceled 12 hrs before going for a cleaning and was told 6 months to reschedule. Call their Lombardi office as instructed and told 6 months. Ask for a supervisor and was told I would be contacted. Had to call again a week later because never got a call back. Once I did get to a supervisor and explained the billing issues, cancelled appointments, potential wrong tooth extraction, no call back, all she said was maybe time to go elsewhere since you are having issues. Never wanted to correct any of the issues. The only explanation she would give me is there is a dentist shortage and there was nothing more she could do. Then stated it would be 6 months before my wife and I could get in. This is the poorest customer service I ever experienced. Would never recommend them to anyone.Review fromCheryl

1 starOne of the worst dental offices I have ever had to deal with. I had an appointment at this office of "Forward Dental" in West Bend Wisconsin. I have been dealing with them for over 1 year now. As of today's date, 11/27/2023. I was in my network for going to my dentist office which was Forward Dental. At the time that I went there to have a crown done on my tooth, they did not have a regular dentist on board. They had to outsource a dentist to come in for at least 2 months until they found 1 to hire on. Before I left the dentist office on the day on my appointment, I made a payment of patient portion of $581.00, and the remainder was for UHC. I paid my portion of the bill via credit card. I could not leave the office without paying my portion. I have gone into the office several times last year in 2022 with the billing statement, and when I went into the office with this same bill, (Cindy) who was the office gal doing the insurance at the time, I think she no longer works there. She told me that she would take care of this. I have called so many times via phone and left voice mails, and not 1 person has ever returned my calls to see what I wanted. I was in my network with going to "Forward Dental" but the dentist they outsourced was not. They did not hire her on to do the work. Therefore, there was no phone call made to the UHC to check on this, and I was never informed, that she was not an in-network provider. Although I did go to "Forward Dental" who is in my network. I was in the right, but they were in the wrong, because now they're trying to make me pay for their screw up of charges for $262.00. I refuse to pay this bill, because, after several phone calls to UHC, I was informed that the whole bill was "PAID IN FULL". So why are they continuously trying to come after me for this balance that they say I owe. I do not owe anything. Let them figure it out. UHC said today 11/27/23 that I only had 1 year to appeal this. I've been appealing this since 2022. FIX THIS.Review fromSarah S

1 starI, too, am going through a months-long insurance/billing situation. While at my last appointment, I was told I needed a mouth guard. I trusted the dentist and said I would like to go forward with the process. I was told my insurance was run and their was a remaining balance I was responsible for. I agreed to pay. I checked with dental-insurance via phone and was told the covered 100% of this claim, "You should have not had to pay out-of-pocket at the appointment", they said. When I call to complain to Forward Dental, they have a list of excuses and unofficial jargon they throw at you to keep you at bay until they can sort out their faulty practices. Just deplorable. The person I've spoken with several times has laughed at me when I've stated how exhausting this has been, and I've been hung up on. I still do no have my money more than a month later. I think Forward Dental hopes their scamming will go unnoticed for those patients who won't follow up with their insurance companies. I just wonder what an employee training session must be like? 1 starI have been going to the Kenosha forward dental office for years. Recently went in for a cleaning and was told I needed a root plane at the cost of 1100.00 my insurance company denied this procedure and I can not afford or want this procedure,but they won't do a regular cleaning for me so essentially saying I can't go there anymore. I have never seen anything like this before trying to tell me what it is I am going to do. So sadly I move on. There customer service is lacking. Worst customer service possible. I've been in an ongoing insurance/billing situation with Forward Dental for 5 months on their mistake. I've been lied to repeatedly and now today they will not take my call and put me on hold for 30 minutes so I would simply go away. Avoid at all literal costs as it seems to be a fraudulent billing company as I have transparency into my insurance account when a claim is submitted...guess what...no claim submitted in 3 months after multiple calls and being told it was being taken care of but yet I continue to get a bill with the wrong amount.ForwardDental

1 starhalecorners to kenosha locations horrible experience with them find out this year i need a root canal/a crown, i am thinking that that someone have messed up my gums/or teeth didn't even forward my dental records to my new dentist horrible service im glad im not with them anymore scared and nervous and worried about this

1 starAbsolutely the worst service ever received, lied to, still won't give me a complete copy of my records, etc. All I want is a refund fr services not received. Numerous attempts to call no return alls. I could go on and on. Don't go there. It's a nightmare!!!!!!!!!!!!!!;
