    I requested records from **** and ******************. CIOX mailed the records, without prepayment. The letter that came with the records said that Confidential Information and attached to the records was an invoice for $198.38 for St. *******. The invoice said that copies of records was processed and shipping cost was included. These were the only records received from CIOX. Once I got the St. ******* invoice, I wrote a letter disputing the invoice. The debt was disputed because the invoice said that the request for records had been processed, and full payment in advance was required and must be received within 30 days and FULL PAYMENT WAS REQUIRED PRIOR TO RELEASE.I received an email 11/2023, with the invoice that had a new invoice for $690.91 for records from ****, along with the St. ******* Records invoice for $198.38. The **** invoice had a charge for shipping costs, but unlike the St. ******* invoice, the **** invoice did not say that our request for copies had been processed, because they never mailed the **** records. I disputed the invoices. **** emailed me and said the **** records were released and payment was required. The email stated that the St. ******* records were not released (even though they were the only records mailed). The email said please note, records will not be released without payment nor will this invoice be waived to release records. On 2/26/24, my office sent me an invoice that was mailed to the office that only included the **** records. On 3/4/24, I disputed the invoices. I received an email saying the St. ******* invoice was canceled, because it was not owed. They stated that I owed the **** invoice. On 6/3/24, **** sent a threat saying the invoice may be transferred to a collection agency. If CIOX mailed the **** records, we never got them. The only records received were the St. ******* records. I have the cover letter that came with the St. ******* records not have a cover letter for any **** records.

    CIOX Health, a ***************** is contracted by health insurers to request patient records from healthcare providers. For unknown reasons, CIOX has been calling my personal mobile number--sometimes multiple times per week, outside of business hours, requesting patient records. These messages are often incomprehensible.I have reached out to CIOX by phone, sent a trackable letter to their CEO, *********************, but **** refused to respond to this concern. If I fail to respond to CIOX, I could be considered non-compliant with health insurer contracts, thus I am obligated to return the calls, thereby reinforcing the incorrect number.I have verified that my phone number is correct with health insurers, with CAQH, but CIOX refuses to use the correct number. I expect CIOX to do the following:- Unquestionably and permanently erase my personal mobile number: ************ - Only use the office number: ************ - Respond and--I hope--apologize for their ongoing and bothersome calls and messages.

    **** also known as Datavant is an independent release of records company. I paid the $20 fee for release of records (I have a receipt). Blank sheets of paper, no records were faxed to my ******* **** company along with a billable invoice to the **** company amout of $48.54 on 1/9/2024. When I called ciox to inquire on the status of release of records; ****** informed me of the invoice listed above sent to **** company *******. ****** stated my records would not be released to them until ******* paid the invoice.I am pretty sure this is illegal since I paid the $20 fee for release of records at the time of the request and blank pages were faxed along with an invoice to *******. ******* will not pay this invoice and stated they have never heard of this.     we overnighted payment to receive medical records. the processed 1/4 checks and lost the others. i reached out to accounting and verified that not only did they receive our checks, they also cashed them already. now they are trying to force us to pay again and won't release our records.

    Our firm frequently relies on Ciox Health to obtain itemized bills and medical records for our clients' personal injury claims. Unfortunately, we have encountered persistent challenges with the accuracy of their processing, leading to unnecessary fees for our firm.Most recently, Invoice Number #********** was processed incorrectly. Despite reaching out to Ciox Health's customer service for assistance, the interaction was hindered by a language barrier, and regrettably, we were hung up on by *************These recurring issues are causing significant disruptions and financial implications for our firm.

    I paid $6.50 for 2 X-ray images on disc for the use of my chiropractor. I received a disc with ***** of irrelevant documents, and no images. Ciox refuses to refund my money
