            ************************** is currently posting my personal information on their site. The site does not have ANY way of contacting them or filing a complaint. There is also not an option to update the information with the accurate details for my business rather than my personal contact information.

            medicarelist.com has a listing for me attached to an old business name that I had to change. I never asked medicarelist.com to make this listing for me, they just made it. There is another business in my area with a similar business name. That business is contacting me telling me I am infringing on their trademark because medicarelist.com has a list for me with old business information. There is no way to contact medicarelist.com to dispute or get them to change any of the information, and this is affecting my livelihood as a business. I am uploading the letter from the lawyer and the screenshots the lawyer has taken of my business information attached to the wrong business name. I need this URL taken down or at least have them put the correct business name with my address, phone number, email, tax id, and npi.

            I am a mental health professional working with patients who are unstable at times. This site is placing my family and myself in extreme danger by indicating my home address instead of my business address. There is no way to contact and ask for this site to update that information. So there is no way of reducing the risk to my family which is causing me severe anxiety.

            This website lists incorrect professional status (I am a psychologist now), also home address and phone number rather than my business address and number. I am a provider working with at risk populations. There is no way to contact this website or correct my own information. This site states that they update from CMS and NPES but they don't. The professional list info is outdated by 4 years and states it has not been updated for 4 years, but they have my personal address and current clients and the public can see this.

            I was searching my name on ****** to determine what information would show up about me and this company Medicarelist.com has listed my HOME ADDRESS as well as my other information. They have no method where I can contact them regarding this issue. The only reason that my home address is listed on the *** directory is because they REQUIRE an address in order to obtain an *** number and I do not have a brick and mortar practice at this time so I HAD to use my address. The actions of this company have put me in ***** way. They are violating my safety and providing no recourse for me to address the issue with them.

            I am a healthcare provider and this website (medicarelist.com) has my personal home address and cell phone listed. There is no way to contact this company. I found a bogus phone number, there is no email address. I want my personal information removed, it is not safe in my profession to have my personal information available to patients. They can leave my practice address and phone number on their website, but nobody has a right to publish my personal information without my permission. This is dangerous to me and my family.
