            I have made several unsuccessful requests in the last year and a half for my medical records to be sent to my current provider. Many months ago, I spoke to a supervisor for Orlando Family Physician, who told me she could not locate my records. After, going back and forth with calls, I was told that they located my medical records and would have them immediately sent out to my provider. Here we are months later and my physician still hasnt received my records. I made a call today, and was told they couldnt send my records out because I am bo longer a patient. The medical assistant said she would reach out to the supervisor, while I was waiting on hold. Moments later, I was transferred to someones voicemail. This is the ******* office in Orlando, specifically that Im referring to. They are preventing me from getting proper medical treatment by withholding my records. Thank you for your assistance with this matter.

            I followed up with my primary doctor after receiving  the response. They have still not received my records. I also attempted to reach out to Orlando Faniky Physcians and have left several I just want my medical records messages and have not received a call back. Also, attempted to be in the automated call back and have not received a response.

            Hello - 

            I would like to respond once again to the complaint that the records were sent to the fax provided on separate occasions. The patient is stating they still do not have them. I show that they were sent again 12/13/2023 at 11:20AM, a total of 96 pages to ******************* at ****************, **.
