      I've been calling and requesting financial assistance paperwork from HCA ******* Mercy Hospital for 4 months starting since September. I get a runaround that it will be sent via ***** (never received), **** (never received), and email (broken link to check messages). I am trying to get my bill resolved with financial assitance but time is running out. I've called several dates last being 1/22/24, 1/17/24/ 12/23, 11/23, 10/23, 9/23. I cant get a hold of a supervisor (specifcally a ************** and *****) telling me that a supervisor will call me back in ***** hours and that it will be designated urgent phone call. No one ever calls me back. No one ever gets me the paperwork. I am EXHAUSTED. I have a bill of over ***** and I need financial assistance that I can't seem to get.

      I called ************** on Tuesday Jan 23 ant 1:14p and asked for a supervisor. Was directed to voicemail and told that Id get a call back ***** hours later. Its currently 2:30p on Thursday January 25th. No one has called me back. Im very angry.

      On January 25th I reached out to the phone number ************** again. **** customer assistant sent me to the voicemail again. Left a voicemail stating Id like a call back. Service was abysmal and very rude.

      As of January 31st, NO supervisor has called me back after leaving several escalated voicemails. Despicable.

      Acct # *********** Acct # *********** Dear *** or Madame,It is with a great source of shame that I share with you that I am homeless. Although I am insured with a plan from the *************** Act (ACA), deductibles and co-insurance apply to ER visits. Two of these were processed under the above referenced account numbers. HCA ********************** has opted into voluntary participation of the ************ Act of *******. Faxes were sent to *****************-727-0834, ************ and ************, numbers which were provided during previous applications. It should be noted that I carefully select my plans, so that the lowest possible burden is generated to providers and that the majority of the claim is actually paid out. Very much being my desire to also support those who do so much for the community, during the past few years, over $20,000 have been paid out, by insurers, to the *** hospital, physicians, radiologists, and labs. To date, no response has been provided. As supporting documentation you will find a Certificate of Indigency, issued by the Miami-Dade County Clerk of Court. Kindly send a letter confirming that the application has been approved, as the radiologist is also billing separately. Although their amount is less than $10, the potential for this to end up in a collection agency is a matter which generates further hardship for someone in my position. The visits were fairly quick and, overall, very little was actually done. The profit margin for the facility should have been fairly high. Once again, it is my desire to support your business because you have helped me so much, and it is very clear that others are also provided assistance. In the ********** is very much my desire to continue to be seen at your hospital. Please provide the correct fax number for any potential future cases where ************ is necessary. Thank you for your assistance in this matter.Sincerely,*************************
