We take all concerns seriously and investigate thoroughly. Our records indicate that this individual has been contacted by the facility and/or its representatives a minimum of 11 times to gather the necessary information to complete the ******* *********************** application and submit these bills to the appropriate agency. Despite requesting it many times, we finally received written consent in February of 2024 and the required bank statements.  It was then determined that this patient would need a Qualified Income Trust due to his monthly income as it exceeded the ******** allowable income. As of today, April 10, 2024 a Qualified Income Trust has not been set up and this account has a very high balance that has not been paid, nor have arrangements or a payment plan been attempted by this individual. Furthermore, we acknowledge that she has concerns regarding her fathers care. However, the facility considers patient information to be confidential and complies with HIPAA regulations. She may contact the facility directly to address concerns regarding your fathers care as it may not be discussed via this medium. The facility considers the health and wellbeing of all its residents top priority and we continue to provide quality care to all our residents.
