      Hello,I am sending this complaint regarding emergency dental care I received on 05/16/24 from the Dental Walk-In Clinic and the care-providing dentist (contractor) found not to be properly registered with a valid TIN or EIN for this business address location. The actual dental care received by the Dental Walk-In Clinic of Tampa Bay was very good. However, my overall experience with this dental facility has been an absolute nightmare! My complaint is that the Dental Walk-In Clinic of Tampa Bay allows their contractor dentists to service patients without being properly registered for that facility location, which in turn may result in insured patients not being reimbursed by their insurance provider!!!Although the Dental Walk-In Clinic of Tampa Bay business does not file claims with insurance companies for payment, potential customers who have insurance providers that would be seeking emergency care or urgent care due to time constraints would not be able to receive reimbursement payment, in most cases.. This is ****************************** providers, including my insurance provider (Delta Dental MA.), require a TIN or EIN (W-9 form) to be registered for all practitioners, both In- and Out-of-Network BEFORE REIMBURSMENT CAN BE PAID.I have attempted to resolve the matter with multiple phone discussions (2-way & 3-way calls) with the Dental Walk-In Clinic of Tampa Bay and my insurance provider. However, the administration manager for the Dental Walk-In Clinic of Tampa Bay refuses to rectify the problem.

      To whom it may concern:

      Thank you for reaching out to us regarding this matter.  In response to customers complaint, we would like to address the following information.  The customer was told from the beginning that our offices and providers are not in network with any dental insurance companies and that all fees for professional services rendered are due at the time of service.  This information is also clearly presented and signed by the customer in the patients Office Policies and Procedures document.  The information states the following, as a courtesy, we will fill out insurance forms for you so that payment will go directly to you, but we cannot be responsible for insurance delays or problems.  We do not accept payments from insurance companies, all such payments are returned to the insurance companies.  We have attached a copy of that portion of the document for your review.

      As additional courtesies to the customer, our team spent numerous hours offering assistance to the customer in hopes of assisting him through the process of being reimbursed by his insurance company.  Again, as previously stated, we can not be responsible for insurance delays or problems regarding whether or not insurance companies will reimburse their customers. 

      Our team provided the customer an itemized and completed *** claims form so he could send to his insurance company for potential reimbursement.  Our team also spoke to his insurance company on several occasions providing as much information as possible to assist with potential reimbursement.  Our team also provided a letter detailing the additional information that his insurance company was requesting.  We have attached a copy of the letter written to the insurance company.

      The Dental Walk-In Clinic does not complete W-9 forms from insurance companies.  It is with regret that the customer has stated that he has no problems with the care that was provided to him.  The problem is with his insurance company and their reluctance to reimburse him for services with a provider that is out of their network.
