              I am writing to discuss my account #********* (Guarantor #*******) with a balance of $8,680.00 related to an emergency room visit on 7/8/2022. ****** from the *** business office has contacted me several times about this balance. I explained repeatedly that my visit was not due to a workmens comp claim, although she said it was initially filed through my ******************** as such. I visited the emergency room because I had Covid, which I tested positive for the next day. Since the visit was not work-related, the balance should be filed through my insurance as a normal emergency room visit.Over the past 4 to 6 weeks, I have tried calling your customer service number *************) multiple times. No one answers, and I am forced to leave messages. Only one message was returned, and I missed that call. Despite leaving several more messages, I have yet to receive a callback. I want to resolve this matter, but your staff has been unresponsive. Your staff needs to re-file the claim as a non-workmens comp claim. The phone number for your staff to call is ************ for *********** and they will assist in re-filing the claim correctly. ********** confirmed (Reference #T20240509205262891) that *** filed the claim incorrectly. To process and pay the claim properly, *** must re-file it with **********. There is nothing I can do on my end.Due to ***'s inability or unwillingness to communicate, we are running out of time to get this claim paid, as it is almost two years old. I am not responsible for this balance due to ***'s negligence in this matter.
