Trying to change Doctor/ Gynecologist but **************************************************** (Optimal womens care) making my process hard for me especially since this is my first pregnancy & was currently told by an other Doctor that Im high risk pregnancy which ******************************************* did NOT inform me . *** called & when to the office many of time telling them I need my records fax to my new Doctor/Gynecologist ************************************* Fax number: ************* Phone number: ************ ******************************************* still havent fax over my records & every time I call her assistance states that she will fax my records by the end of the day, which is a lie. I wait a few days & call my new Doctor/ Gynecologist office *************************************************** at ************* state that they havent receive any of my records . Its been over 3 weeks and Im still trying to be seen by my new Doctor/gynecologist but cant been seen until they get my records. & I did go in & sign release of information form & wrote down the new gynecologist name, number and fax number. ******************************************** at optimal Womens care located at ************************************************************* are very unprofessional.
