      date of service 8-6-24-requested an itemized statement so I can be reembursed from ********. The statement needs to be date of service, service performed (hearing test $60.00, cleaning of hearing aides $60.00). I called them left a message with ***** on 8-7, 8/12, 8/13, 8/27 and never received a response. I left message with ***** supervisor ***** ******** on 9/10 and again no response. I just want an itemized statement or a they can put it on letter heard listing the information and mail it to me.

      In January, my son started having ear pain and decreased hearing, so I called and spoke with a nurse to see if he should be seen by ENT or by his ***. She stated that since he had a prior surgery there, he should be seen. We were scheduled for the next week. When my husband took our son in, he was only seen for a hearing test and told he had fluid behind his eardrum. They said he needed to be seen, but that they couldn't see him until 4 weeks later. We scheduled the appointment but took him to the *** so that he could be treated. The appointment came time and the day of we were contacted and told that the provider was out so we couldn't be seen. They tried to schedule us another 4 weeks out. By the time we were finally seen by an ENT provider, they called an hour before our appointment to tell us that he would need his hearing test redone because it had been too long and they would not be able to do it that day. I have been contacting billing since January; I have repeated my story a dozen times to people who claim they will have a manager return my call but NO ONE has reached out to me. Recently, one of the representatives I talked to said they had no notes on our file. There was no reason for my son to be seen for just a hearing test if they weren't able to see him and treat the issue. This is poor customer service and a lack of professionalism when a manager will not return your calls. I have been professional and kind to everyone I have spoken with. We will be taking our care elsewhere.

      My son needed surgery. It was scheduled two months in advance. One week before the surgery I was notified that my total after insurance was estimated to be $1770 and that it was due the day of surgery. If I couldn't pay it my son would be denied surgery. I had one week to come up with $1770 and it was an estimate! I managed to borrow the money because my son needed the surgery therefore I had no choice. After my insurance processed the claims, I found that the estimate was over $1000 off in my favor. They owe me just over $1000. I have called several times and continue to get run around, excuses and lies. I've been given two dates that my check will be in the mail and yet the check has still not even been printed according to the last person I spoke to. It's been three months! I want the money I am owed!
