    02272023 / QCKinetix,6420 Dutchmans parkway suite 375, Louisville ky 40203, Talked with ********** Manage. Checking on services she agreed to make a payment scale. Using my card to make a payment, she ,Lora instead of a one payment proceeded to take out $6,500 dollars, full pay.without permission to do or unauthorized to do so , dishonest and fraudulent practice. Need help to get money returned. She discriminated because of my age .she thought I would’n’t catch her dishonest procedure My age 84. When I called her she said we don’t do payments! False,. checked further and found this was a lie on her behalf. All I want is the return of my $6,500 dollars. QCKinetix is from my experience not an honest institution. Thank you .

    My name is **** ****** * and I am emailing as a follow-up to our previous phone conversation that took place on 3/29/23. I have attached a copy of ******** ******* signed receipt and consent forms. Additionally, I have summarized our conversation below:

     

    ******** ******* signed on as one of our patients on 2/26/23. She received her first injection that same day as that is our protocol. Preceding her next appointment, *** ******* called us to let us know that she was already feeling relief and did not wish to continue receiving injections. She informed my office that she would be contacting her bank in order to get a refund and would come in to pay us the amount of the first injection. I do not know what came from her contacting her bank. She later called the office again, speaking very harshly to staff members. I was informed that she assumes we took advantage of her age. The average age of our patients is 76 and our services are intended for this demographic. Please let me know if you require any additional information.
