    Date of the transaction: 12/13/22 & 12/27/22 The amount of money you paid the business: $200 cash for massage services + cash tip What the business committed to provide you: Two 1-hour massages with documentation of completed services and payment receipts. What the nature of the dispute is: The business only provided a receipt for one massage service and claimed the other $100 as a tip, when tips were provided to the business during the second massage. I need documentation of completed massage services. Courteney promised me that she would provide documentation that I can submit for reimbursement of my $200 for the services but hasn't fulfilled her promise even though she's been paid. The insurance requirements for submission of reimbursement include: An itemized bill from the provider of service, listing dates of service, session length, services provided and rate per hour. Whether or not the business has tried to resolve the problem: Courteney has provided two one hour massages, but hasn't fulfilled her responsibility and promise to provide me with documentation for submission to insurance. I chose her business because she uses massagebook.com, which says it is compliant for insurance, billing and FSA/HSA reimbursement. If she cannot provide me with the documents promised for reimbursement, then I want a refund of $100 for the second massage that she hasn't provided documentation for.

    I have added as many screen shots of text message conversation that I can. I have the Massagebook.com files, receipts as well as SOAP notes of his suspicious behavior. This client had been flagged after his December 27th interaction for inappropriate behavior. I seem to be dealing with retaliation in regards to his inability to return to my establishment. I'm afraid he is mentally ill. I have additional images I would like to submit to support myself.
