      ********************* <******************>Thu, Jun 6, 12:44PM (6 days ago)to billing I am writing to express my dissatisfaction with the billing practices I encountered following my recent visit to CityMD *******************************, ****************************** . I visited your facility on April 24th, 2024, for my annual physical check-up. Additionally, I had paperwork from the ****************** non-profit organization that required a physician's signature as proof of my physical test. Before my appointment, I specifically inquired with the front desk staff if the paperwork signing would be included as part of my annual physical check and billed accordingly. The front desk representative consulted with Dr. ********************* and confirmed that it would be billed as part of the annual physical check. After my appointment, I paid the $35 copay for the annual physical visit. However, on May 3, 2024, I received an additional bill for $265. Upon contacting my insurance company, Aetna, I was informed that they cover only the annual physical check and not the signing of additional paperwork. It is clear to me that I was misinformed by CityMD staff, resulting in this unexpected charge. I have been a loyal patient of CityMD Park Slope for years, but this incident has caused me considerable frustration and disappointment.

      Hello,I would like to lodge a complaint for unfair business practices from CityMD **************** on 08/02/2023. I went into this business solely for a covid test and have been unfairly charged $100 for an 'office visit.' The Covid Test offered from this practice were free at the time and I have still been charged. Not once during my visit was I ever told that covid tests were a charge, in fact the extremely disrespectful staff member working there just pointed to the kiosk to self-check in without offering any acknowledgement of my condition. There was no information on the screen to communicate of charges for this service. I even paid a $35 co pay for the visit as I was desperate to be seen as I had horrible symptoms from the covid virus and sought the help from what I thought was a reputable business who puts patients' needs in front of money. Sadly, I was very wrong. So not only did I pay the $35 at the time, which I asked and was told that would be the only charge, I was then shocked when I received another bill for $65 on top of this. In no way will I ever pay this. I have called CityMD multiple times and they have been very unhelpful, with the line either dropping out, being cut off and not picking up. Similarly, they refer me to my health insurance, however it is not a matter of coverage when the procedure should've been free in the first place. ************** at the time issued the free covid tests and specifically named this business to attend. They should've been transparent in the fact they were now charging for covid tests and at the very least told me at the time and I would've gone somewhere else. This is bad business practices and deceitful. The date of service was 8/2/23. Reference ID#*************. I am seeking the invoice amount of $65 to be cleared please. As compromise, I accept the payment of the $35 I already made however obviously would appreciate that back as well to resolve this to satisfaction. Invoice attached.       Please see complaint ID# ********. This is a followup/contintuaiton I am raising this complaint again as it has not been rectified and I have been waiting for a promised update and reply from CityMD for over *************************************************************************************************** not. Also, the issue with my insurance being updated has not been rectified nor have they sorted my last bill. Most important the horrific aftercare or lack of has left me disturbed and traumatized. I've had to take notes from CityMD and share them with another medical provider just for a necessary prescription. CityMD would have known this if they invested in the health and well-being of their patients rather than demanding money from whomever wherever. *************************** from CityMD was supposed to get back to me. If she has left or no longer works for that department then it should have gone to whomever handled her cases it has been over a month and I have been forgotten about.

      Hi,On May 17th, 2024, I visited the CityMd in ****************, ******** ** location concerning a very bad cough that I had. I arrived before 6pm and checked in for my appointment. After waiting over 30 minutes, I was taken in the room and had my vitals checked by the medical assistant. I mentioned to them that I had a cough since Sunday May 12th, 2024, a sore throat and my ears felt irritated. I waited in the room until the doctor was supposed to see me. Everyone I interacted with before this was very pleasant. The person who walked into the room after the medical assistant left was not. The person walked into the room and did not address who they were. I was examined in less than 3 minutes. I felt like my visit was rushed. I had to inquire about my lungs which was a concern for me because I have asthmatic bronchitis. This can cause a really bad flare for me.When I asked this person what kind of decongestant syrup they were prescribing me, they couldnt tell me. I asked if it was ********** and they said no, ********** is for cough. How can you prescribe medication and not tell me the medical name? How does this person know if I ever had a bad reaction to the prescribed medication? Why did this person, not address who they were when they walked into the room? Was I seen by a doctor? I have never had this experience before in my life. A doctor is supposed to address themselves when they walk into a room. Im not sure who I was seen by during my examination. Anyone can put on a lab coat and walk into an examination room. This is unprofessional and borderline malpractice. What happened to the bedside manner for doctors at CityMd?This reminded me of a different visit I had at the *********** location where the doctor asked me what medication has worked best for me. I am not a doctor. I am paying you for your medical expertise and to prescribe to me what is best for me.My discharge paper states a persons name but I am not sure if that is the person I saw.

      This is a follow-up to previous Complaint ID # ******** entered on 4/5/24 (original below). I was contacted by the business office, who noted that they were reprocessing my claim. I then called and spoke with a representative who assured me the matter had been resolved and emailed me a statement showing I did not owe them any money (attached), so I marked the claim complaint Resolved. However, today I received another bill for the same service. I would like this to stop.ID # ********** went to ********************s ************ location on 11/26 after cutting myself while cooking. I received good care while there. My issue is with the billing office. I have made multiple phone calls, including a 90-min call in which I was on the phone with both the Summit Health billing rep and my insurance company rep. ******************** urgent care was in-network with my plan but it was applied to my OON deductible. I received a bill for $219 for the visit. Eventually, after many calls to my insurance, it was determined that Summit Health put the wrong location code on the bill. My insurance company called them to have the bill corrected and resubmitted. During that time, the bill has been sent to the wrong insurance company, I have been billed again for the same amount, and now they have sent me an even higher bill for $616. I shouldnt owe anything for this service, but they continue to make billing errors and try to collect from me.

      Today 05/07/2024 around 3:50 pm I was arrived citi md *********** citi ************ south location for emergency with complaints of hand numbness, dizziness, nausea, and tremors. And I requested immediately emergency room because Iwas feel so bad. But your couter worker start to register to me i give insurance card and id and he did not take me to the emergency room and make a doctor *********** 30 min for me. I dont wait because i was so bad . I take taxi and going to another emergency room, Doctor diagnosed blod pressure problem and take care. Your service is very bad, Aiming to make more money, putting human health in the background, You are an institution that operates with an unethical idea.I will share this complaint on all platforms ***********************************

      On 4/10/2024 I took my 12 year old *********** (4/28/2011) for a physical at CityMd and I was told it would be covered by my insurance. I am now being billed $300. My insurance company is saying that physicals arent urgent therefore they arent covered at CityMd. CityMd lied to me, proceeded to do her physical and now is charging me $300. They were dishonest and took my insurance information knowing that it wouldnt be covered. Yearly physical exams are free!!! Not even an ER visit ***** is 300.

      I went to city md located on *******, **, with my toddler son to have his finger looked at after a door closing on his finger. I was not home at the time and initially thought his finger was caught when a car door closed (wife not fluent in English). I paid the copay for my health insurance but was later told by the manager, *****, that it needed to be filed as motor vehicle accident and that it will go through my car insurance. When I spoke to my wife again, she said she never said car door and that I mustve misheard- the door is not from a car but from a bedroom door. When I relayed the message to *****, she stated since I initially stated it was from a car door, that it cannot be changed. I tried to explain what happened, that there was a misunderstanding between my wife and I due to language barrier, but she still insisted on filing through my car insurance.

      The CityMD at ************* has displayed a high level of incompetence that is about to cost me my education. A lab result which was supposed to be given to me is taken a longer time and they lied to be the first time that its available when its not, after wasting time, I was told it is not available, I came back the 7th day again and still the result is not available. This is is going to cost me a lot if I dont get it before Monday 1st of April.

      On 01/10/24- went to city MD for the following symptoms: fever, chills, cough, chest pain when coughing, sore throat and mucus. I was told I had a 101 fever and I was told that I had to be tested for Covid/flu. The practitioner comes in and after I tell her all my symptoms, she immediately says I have Covid or the flu, she listens to my breathing, looks in my ears, and that was the extent. I told her that I had a sore throat, but she did not look in my throat. Leaves the room and said shell be back in 15 minutes when the Covid test comes back with results. The Covid test came back negative. She walked in and said I came in too early for that test to show that its positive and I need to come back in when Im sicker. She wanted to send the culture out I told her I did not want to do that as I dont believe that is what I have. I expressed to her that maybe its bronchitis and I would like to have an x-ray done but that wasnt possible since the x-ray tech left even though they were still open for another 2 hours. I was sent home with prednisone and an inhaler. Went back on 1/14/24 as I was not any better. My throat had now developed white pus within my tonsils and on the roof of my mouth and was in excruciating pain. Thought maybe it was strep- they did a culture and it came back negative. Again was dismissed with viral pharyngitis can cause all of this and when asked why I could not receive antibiotics his response was If you dont have it, you will receive negative affect such as diarrhea and stomach pain vs actually treating you. Was sent home again with more prednisone. Was able to follow up with and ENT on 1/16/24 and the ENT was shocked that I was not give antibiotics as well. He prescribed antibiotics they took a culture of my throat came back positive for mold infection. Ive uploaded my medical record/results from my ent and pictures of my throat.
