  My mother is currently in the *********** WV facility. She had a CNA that was aggressive in changing my mother. Mom reported her to a nurse on shift, which brought the social worker and name tagless Charge Nurse Jackie into mom’s room, to what felt like an interrogation. The next day the same nurse threatened me with police, because a CNA improperly cleaned my mother’s butt leaving feces on her and I let CNA know she was not clean. . My mom was improperly dispensed medication, the nurse laughed and walked off when confronted. She has developed bed scores and has went up to 10 days without showering. The facility refuses to get her up to assist in walking. The food is inedible and often cold. There is a daily menu provided, but seldom is the food served as listed on menu. If you complain about lack of care, they will send social service to your room to try to intimidate you to move to another facility. This place is horrible and the dementia residents seldom receive appropriate care. If you love your family members, this is NOT THE PLACE.!

  don't put your loved one in this terrible place they don't take care pf their patients they lost my husbands clothes they also set him out in the reception area with only a diaper on so everyone could see hhim knocked out slumped over with his mouth hanging open. also they let the patient walk the hallways and get in other patients beds and half the time they don't know where they are. my husband was 85 he died in there after 12 days was in good samaritan for 5 weeks and no problem i feel they are responsible for his death shame on them. and they have no clue on billing. and the billing area doesn't care weather he is cared for or not they just want to charge you with no care just room and board. I pray for all those patients that need care because they are not going to get it here.

  This place is absolutely horrible! My dad was only here for a mth and they almost killed him! He was there for wound care, and the wound got worse!!! He got an infection, a UTI, was severely dehydrated, and constipated! They are so negligent! I had to call 911 to get him medical attention! My father is now in the hospital recovering. He will NOT be returning to that place!

  I can't even give 1 star!! My husband was in that place for only 6 days. Went in on memorial day 2022, me and my daughter went in everyday. Things were not right. Multiple occasions had to request someone ck O2 level for him because he he was strugglung breathing and he was on oxygen. Next day his hands were like ice, after that i had asked to have a nurse call me about concerns. I had talked to 3 different people on 3 different days and those records from those days are missing from his medical record. On the sixth day i received a call saying my husband was taken to hospital because he was unresponsive!! PLEASE DO NOT SEND ANYONE TO THIS PLACE!!!!!!!

  My husband needed 30 days of acute rehab. I took him home after 11 days - Never got treatment plan.Never contacted by anyone regarding his care. He layed on a cot, not a hospital bed. He is not supposed to lay flat due to respiratory illness.Never received PT, OT, or SP No oxygen until I complained on day 5 Deplorable conditions.Disrespectful, incompetent, rude staff Patients in hallway all day Patients allowed to smoke and play loud music from the patio deck. Room was tiny and dirty, water on the floor for day.

  OMG, **** * I had the same experience. The hospital recommended this **** hole for my husband who needs acute rehab. He has been at **** ***** in Baltimore for 3 days.The first day I went to visit, we were yelled at by the staff and disrespected in every way. His clothes disappeared on day 2. No one has contacted me about a plan of care, or to update me on his progress. The place is filthy, staff is unprofessional ignorant, and dangerously neglectful. The oxygen he needed didn't work for 3 days and only fixed after I insisted. The food is not fit for animals and not at all nutritious. I'm told to speak with the social worker on Monday. I am trying to get him out of there asap. They should not have a licence to operate, anywhere. My rating is negative 100 stars.

  The worst nursing home in the history of the world. My dad was in there while we tried to get him on medicaid into a better facility. The staff is incompetent all of his clothes were stolen multiple times. do not ever put your loved one in this facility.

  This corporately owned facility in St. Louis Mo. (Green Park nursing home) is beyond bad to the point it's referred by my family member as well as many other residents as "a slumlord run facility". It's pretty bad when you catch the staff falsifying/charting records wrong and having the nerve to shove the document in your face to the point you present them with your own pictures and video evidence stating otherwise. Then mysteriously the surveillance camera gets stolen and ripped right off the wall and completely tearing off the drywall right down to the metal studs. in fact it's become so bad that I had to call 911 on the staff members twice (one of the them started pushing me. I've attempted to go outside the customer complaint number and contact the higher up's myself but to no avail because the staff won't even call you back, ignore you and not even acknowledge your existence. Not only do I stand by my comments/statement here If the cooperation even attempts to rebut my comments I will be more than happy to present my time stamped documentation and notes the the BBB so than can confirm and authenticate the my claims.

  Three rivers healthcare center is hands down the worst nursing home this side of the Ohio river. Not only is the building falling apart but the “managers” who run the building are rude, liars and incompetent. My mother was here and they were of no assistance after she passed when it came to getting her clothes and belongings. Tomek O****** the administrator is the most arrogant, passive aggressive “professional” I have ever met. DO NOT SEND YOUR LOVED ONE HERE IF YOU CARE ABOUT THEM!
