I, ******* *****, checked into Landmark Recovery of Indiana on June 1, 2023 for mental health. On or about June 23, ************************************************************************** ***. After being informed he was to be placed on hospice and dying in a matter of days I told Landmark I was leaving to attend to my father.They then proceeded to threaten me with categorizing my file as "against medical leave" if I left. (AMA). They accused me of fabricating an excuse to leave the facility and knowing before I checked in that he was "ailing". He was not dying when I checked into Landmark and only under physicians care for ********** father sustained physical trauma causing irreversible damage and accelerating his health decline leading to the *** and eventually hospice and death within a week.Landmark refuses to do a simple coding change so my insurance company will pay for my treatment at the Landmark facility of ************. (****************************************************).I have been in contact with their representative/ Director of Compliance, ***** ****** Direct ************* Email ******************************** Corporate address *********************************************** Appeals have been made regarding getting my insurance to pay the bill with the last communication thus far being the attached bill and internal appeal response dated March 14, 2025.Death certificate for my father is also attached.Initial Complaint

My son was referred here for rehab and everything was well until he was admitted! Once he was admitted everyone stopped Returning my phone calls or any emails. I am trying to get documentation that he was admitted for proof for the courts but no one has contacted me back and I also would like to know how my son is doing. I understand there are certain things they cannot inform me about due to h IPA a laws but it would be nice to know how well he is doing and put my mind at ease As a concerned mother. I do understand that the first 7 days he is not able to contact anyone but these first 7 days of rehab are the most crucial and are a higher risk of something medically happening so I do not see why they don't have a plan in place to update parents Or loved ones that are at their facility. And now I am reading all of these bad reviews and horrible things about that facility I regret Sending him there because I was told they accept ******** and when I went on their website it said they do not accept ******** so I called before I sent my son there and was insured they do accept ******** and they needed to update their website so now seeing all of these reviews about being billed or my son will be billed Because of this neglectInitial Complaint

I was required to enroll in an Intensive Outpatient Program this year (IOP). When I arrived for evaluation, I asked about insurance coverage and was told that someone was working on it, not to worry. I wasn't given any information or updates until after IOP was completed, and they tried to bill me for out of pocket treatment stating that my insurance company didn't have a contract with them. I never would have completed a program that wasn't even covered as out of network.Initial Complaint

date: Feb 2-7, 2023 during intake they took my valuables put them in a container with a list of the contents of said container. they took my phone, my house keys, and wallet which included all my cards as well as $78 in cash. I was hesitant, but was promised i would see them lock it in the container so id know it had to be staff if something did come up missing and that it would be locked in a cabinet only accessible to the man in charge of the whole facility, so i reluctantly gave up my belongings and signed off on it being locked up in the *************. the lack of care and my meds being given to me so sparatically caused me to have a seizure where i was taken to hospital. hospital staff wouldnt send me back to finish my ten days because they doctors call the place party center of america because of the number of er patients they treat from overdosing at the facility way after any chances of that being a possibility, and were surprised to hear i wasnt there because i was high but because they didnt give me my meds when i should've had them i wasn't told when it was time to eat and only had 3 cans of soda and thats all i had for those 5 days. I was so weak nurses had to hold me up to do any test the social worker told me i could go back home and should definitely choose a different facility and when i did i went back to collect all my belongings. theyd brought it out in 2 trash bags instead of the bags they were packed in, and told me itd just be a couple minutes for the boss to get my locked away stuff. i waited so long i started to question the size of the trash bags in comparison to the amount of stuff i had and as soon as i opened the one bag the boss brings out my phone wallet and keys opens the container in front of me so i didnt think to check to make sure my cash was still there i get home they kept most of my clothes my winter coat and my cash gone tried calling phone always busy and chat agents stopped responding once i said something to them about itInitial Complaint

This is regarding a previously submitted complaint ********. The complaint was closed before I was able to submit information about what I had heard from the company in discussion. This is the response we received and we promptly sent our address. I was at landmark recovery in Las Vegas Nevada January 21, 2022 until February 1, 2022. On that last day they collected $2500 from me for aftercare which they told me would be about a 30 day program on its own and after returning home find out that they were not able to help me because I lived across state lines and it had something to do with a Law regarding electronic video conferencing. I have been attempting for the last eight months to recover my $2500 and I have had very limited communication with the facility I'm sure they are aware of it but they have not issued a check or a credit to my account and I'm very upset. My relative was admitted to Landmark Recovery of ******** on 8/7/21 at 6 pm. A check in the amount of $2605.25 was paid on 8/9/21 to cover the medical detox (estimated 5-7 days) . On the evening of 8/9/21, he was transferred to *********************** for more aggressive management of his substance use disorder. The facility was unable to manage him per their protocols and he had to be transferred or picked up. The facility submitted claims to our insurance for 8/7/21 and 8/8/21 with our insurance covering everything except $650. We should have received a refund since he did not stay with the anticipated duration and our insurance covered all but $650. The facility owes us $1955.25. We have attempted to discuss this with the local billing department who referred us to the corporate office. We have submitted online requests and spoke with someone at the corporate level who told us they were still processing the refund but this was several months ago. We have not received a refund to this date.Business Response

I checked into Landmark Recovery on Tuesday May 31st 2022 around 9pm or so...where I paid $6,000 Dollars Cash upfront for 8 days based on the services landmark was going to render me. and on Thursday June 2nd 2022 I was rushed to the ** from landmark recovery due to a mixture of medicines I was given at the facility that landed me in the hospital which ended my stay at landmark! I did not stay the full 8 days...since then I still have not seen any of my money returned to me from landmark wether it is to be prorated or given back in full. I have talked to *** a manager at the facility I went to, who keeps giving me the run around saying "she's waiting on corporate to answer her" well how long is that ? It says they have a 30 day return policy and it's been well PAST the 30days so where is my refund? I've called the corporate number and of course you can only leave a voicemail no real person ever answers & to which I have left voicemails as well. I did not stay the full cost of my treatment so I am due a refund!!!Customer Answer
