    I contacted the office of **** ********* MD with High Plains Rehabilitation Association I spoke to a Jennifer and also a Shawn they both declined to give me their last names. I was needing some information regarding my medical records. I have been going there since early 2003 -2004 and requested them to fill out a form that was required for a job I was applying for. It was a very simple form to state I had no physical restrictions on me. I was told by the first person (Jennifer) that they had no information on me. I have been going there for over 10 years. I also have a bill they just sent me for my August visit but she said they had no information on me. And if I wanted the form filled out I had to make an appointment to be seen by the doctor. I was just in there in August 2022 for my wellness check up. I go in every 6 months and last time I was in there I was told they had a new policy and I would need to be seen every 3 months instead of every 6 months. Even though Dr. ********* said for me to come back in 6 months. I agreed since I felt like I had no choice. But then she told me I had to be seen in order to get the form signed. Both Jennifer and Shawn told me the Office Manager had to approve this form being signed. I was upset since this is my information and my medical records. I have always paid my bill there and have always done as they asked. So I requested all my medical records so I could show those to my new job in the hopes that they would accept that. I was told I could pick those up 9-29-22 then I was told I could not pick up until Office Mgr. was there. It is a *** ******* position with the ****** ******* Appraisal District. This is my doctor and I have been with him over 10 years and now I can not even get him to sign a paper regarding my records unless I pay them. I thought that was my right as a patient. They are not even following the doctors orders about the 6 months the front office is practicing medicine on their own by changing his orders.
