Reference: ****, dated 9/25/2022, Account *****, regarding medical treatment at location UCare Urgent Care *******, ** on date April 10, 2022, for $335 amount. Summary: This charge is covered by my insurance plan, US Family Health Plan, via *************************; a copy of my insurance card is enclosed. This information was provided to the UCare medical office when treatment was received and via follow up calls.Action Required: Please immediately file a claim to my insurance provider. They are standing by. As of 3 October 2022, USFHP has no record of a claim for this charge submitted by UCare billing. US Family Health Plan *************************** ********* Submit to:P.O. Box ****** **********, ** ********** For questions call USFHCP **************/************ History:On April 10, 2022 I sought medical treatment at UCare Urgent Care in El Paso, TX, and at the time, I provided my insurance information in person. In late July, I received the bill dated 7/23/2022 from UCare for $335. On August 1, I called UCare Billing at ************ and spoke with ****** to ensure that the correct insurance information was on file to submit the correct insurance claim. On September 1st I received a copy of the same billing action dated 8/26/2022. I called USFHP and spoke with *******, who verified that the visit with Urgent Care was covered and that an insurance claim from UCare had not been submitted. On the same call, ******* from USFHP called UCare at ************ and had a conference call with *******, who ensured that the correct USFHP insurance information was posted to the UCare account so that claim could be filed with USFHP. On September 30, I received another bill dated 9/25/2022 for the same billing action from UCare again.On October 3, 2022 I called US Family Health Plan and spoke with *****, who verified that UCare had not filed a claim with USFHP.I received an additional bill October 29, 2022, which indicates this case has still not been resolved.
