In the months between February through April, I am writing this letter to complain to you about the poor service provided by Liberty Renal (U.S. Renal Care) 3330 N Galloway Ave#160 Mesquite, TX XXXXX. This all started when Ruby one of your technician handle me in a very sexual way and I requested that another tech sees me. (Ruby has multiple complaints from other patients) As this went on, I requested a transfer to another facility to the social worker Adelbola (a.k.a.Toya) Lavandier and nothing transpire so then I took my request to the administrator Shawn, again my request was ignored. A technician Rolonda stuck me 4 time in one treatment and still could not get my access, so I had to rescheduled my dialysis treatment for another day. So, I asked four technicians at Liberty US Renal "are the tech clinically trained?"  Each (4) techs told me "no they just got me off the street, I did know anything until I came here." The techs are not clinically trained. RN Nurse Weldon, inserted my portal incorrectly causing a massive rupture which made me see an emergency doctor at Dallas Vascular Center: Suresh Margassery, MD, 1222 N Bishop Ave #500, Dallas, TX XXXXX to have a stent placed in my left arm so that it would not pop and bleed to death. They left me unattended on the dialysis machine for a long period of time with no one in site and kids running around unsupervised. (I have proof of how I was treated at the facility.) I was banded me from the entire US Renal Dallas area by Shawn and Adelbola Lavandier, now I can get my transfer on April 12, 2021. I have had my access for one year without any problems with receiving dialysis until your employees made an error. This is not something that can be over looked and should be dealt with immediately.
