Hello, I need to report Joshua M*****, DDS for fraudulent and unethical billing practices. Joshua M*****, DDS Family Legacy Dental (FDL, LLC) *** * *** * ****, Orem, UT 84057 +*********** I specifically asked for a regular routine exam. Joshua's practice, unbeknownst to me, took additional x-rays that I did not order. When I received the surprise bill, I immediately contacted him, but he refused to acknowledge that he billed me for unrequested services. Instead he blamed it on the insurance company. I have NEVER had a problem using any other dentist before with the insurance company. In addition, the bill clearly shows that Joshua's practice billed for additional services beyond a routine exam. I was neither informed of nor shown any additional services before hand. The services that were billed without my knowledge were D0220 and D0230. I looked at google reviews and I found similar practices among multiple patients. ***** ******* - I received funding from a state program for dental work in combination with my insurance and they clearly communicated multiple times that I am not supposed to be charged for the initial X-ray and exam yet to my insurance was still charged when I called multiple times to try to rectify the situation they blamed me and had an extremely unprofessional and rude and condescending attitude... ******* ***** This clinic said they don't take my insurance and that the only place that did was placed in Salt Lake City and because I couldn't get to SLC they dare tried to charge me almost $500 before the doctor would prescribe me anything further for pain or remove the teeth. I called my insurance company and come to find that I'm fully covered... ***** ******** - ...they will insist on a deep cleaning that insurance will not pay for.... Please help me get my money back. Thank you,
