            I have two daughters, I took to this place to have their teeth cleaned, and the one charge for the fluoride, and the other one was paid under insurance, but since it wasnt paid under insurance, they charge me separately knowingly that my insurance shouldve paid for it and knew this before this !

            I was charge for fluoride, and I never signed or asked for any of that! And being rude about this as well when I told company that I did not authorize!

            Good Morning ******************,

            I spoke to you last week regarding your daughter *****'s balance. Below I've attached her ledger as well as the consents signed by you on 06/22/2022 allowing us to perform the exam, cleaning, X Rays, fluoride & sealants on her permanent molars. As a courtesy we have billed insurance on your behalf. Payment was received 07/07/2022, the balance remaining is your coinsurance. In good faith, the services were performed and we have been patient awaiting reimbursement for those services. Please remit payment in full or do contact me for payment arrangements if needed. If an agreement is not made in terms of payment, we will have no other choice but to take further collection actions.
