I wish to file a formal complaint against a case worker at Madison Hospital for their repeated actions of reporting our family to the ********** of *************** (DHR) every time I request their assistance in contacting the insurance company to arrange transportation for my terminally ill mother. This situation has become distressing as it appears that they are misusing the system by unjustly reporting us for neglect when, in fact, I simply lack the means to provide transportation for my mother. It's important to note that her insurance company offers free transportation services from the hospital to our home, which we have a right to access.The issue at hand revolves around the case managers actions. They have contacted DHR during my mother's hospital visits, specifically in August and on October 30, 2023, solely because I requested their assistance in arranging transportation. It's worth mentioning that I would willingly arrange transportation myself, but I was informed by the insurance company that this task must be coordinated by the hospital. It was so bad I was told not to have my mother admitted to that hospital in the future.
