To Whom It May Concern,I am writing to formally dispute an inaccurate debt collection entry on my credit report. Upon reviewing my report, I noticed an account reported by **********************, listed under account number *********, I believe this information is incorrect and request its removal or correction under the Fair Credit Reporting Act (FCRA).The reasons for my dispute are as follows:The debt does not belong to me.The debt was already paid or settled.I am requesting that you investigate this matter and provide evidence verifying the accuracy of this debt. On December 10 2024 I noticed that USCB America/ ****** Permanente S*** is reporting a illegal, fraudulent unverified able collection on my credit profile with account number- ******** with a balance of 1015 and a date of open March *******. I am requesting that USCB/ ****** Permanente *** delete, remove and suppress this collection from my credit profile. I also noticed ****** is medical debt and that under Consumer Protection for Medical Debt Collection medical debt can not reported on credit report. Also under FCRA 605B once USCB America/ ****** Permanente S*** has to remove this fraudulent collection from my credit profile in 4 days.Business Response

On June 10, 2024 I sustained an injury (********) while performing Physical Therapy duties for my then employer, Meridian Home Health Care. I presented to the emergency room at ******************* in *************, ** on 6/10/2024 and INADVERTENTLY used my private insurance (Blue Shield) when presenting payment. The matter should have been handled 100% via the Workers Compensation construct and a Workers Compensation agent was immediately secured (***** ********* at ******** *************************** who began intervening on my behalf. More than one dozen phone calls were placed to both Blue Shield and Eden ******* ******* with representatives from each of the above ascertaining that the matter had been resolved in full and that in fact I was NOT responsible for any portion of the cost of the care received at *******************. I have recently been contacted by USCB America in an attempt to collect payment. Escalation to the legal arena will be mandated if resolution is not forthcoming.Business Response

This is a fraud issue. The collector knowingly had the wrong number and person provided. Continued to harass me even knowing that they had the wrong person. Refused and stalemated.Business Response

We are in receipt of the complaint and have reviewed the associated call. USCB did not knowingly call the number of the wrong party. We placed a call to a number provided to us in which turned out to be the incorrect phone number. We have since removed the number and have no longer contacted them.

I have recieved multiple calls from this number once weekly for the past few weeks. I googled the number and it appears to be a debt collector for ****** Permanente. I have not recieved any written correspondence from said company and the last billing summary communication recieved in the mail from ****** Permante was a bill for my daughters genetic testing which has been paid by the insurance company. It was not itimized. am no longer a member of *********************** since 1/1/2024 and have been unable to access their online site as a result. I did return the billing summary back to KP in ************, MD per the directions on the notice. I hand wrote if there is anything due to send a written itemization of the original debt or to cease and desist. I have never recieved any other communication. Not via phone, email or mail. am requesting not to be contacted again.Business Response

This company has been harassing me nonstop. I am in communication to get my payment modified by ******. They were alerted by ****** to stop, and still keep harassing me. I was disputing a ear wax removal treatment because it was not performed but they still charged me.Business Response

USCB America has been paid for a collections already and it was done by the Department of veteran affairs, my credit has been attacked relentlessly for the last few yrs and they claimed they would send me a paid in full paper in the mail and never received this, they are now continuing to claim they never received any compensation, this seems to be a very common thing to occur with many people leaving similar reviews with their insurance companies not sending payments over to them. I want this settled at the lower level before taking this up with my congressman **** ******.Business Response
